0059012903 2014-10-06 Production Wells No Violations I am on site today with Kris Anderson to go over the clean up of the condensate spill. The area between the line heaters and the production equipment is where they are excavating today. The spill followed the production lines under pressure back toward the line heaters and out the side of the containment dike. The corner of the dike has been removed and the standup separators have been removed so the area under them can be excavated and the contaminated soil removed. The area excavated now is 12 ft. wide 28 to 30 long and 10 ft. wide. The area during rain events remains covered. It could be another day before the entire spill is removed.

While waiting to witness the cementing of the surface casing I found out that there was a problem with fill up and the casing would not go to bottom. I waited to see if the problem could be corrected and the casing cemented. After 1.5 more hours I advised the operator to contact me if casing did go in the hole.

0740348933 2014-10-02 Production Wells No Violations I am on site today to review the clean up of the condensate leak below the production tanks and dike liner. The left side of the dike wall has been removed and the contaminated soil removed down to 1 ft. below the depth of the leak. It is 4 ft. below the production liner and then 12 ft. wide to the left of the entrance and 3 ft. wide. The line now will be moved above ground after the repair and the dike wall and sides rebuilt and covered with the spray on liner. Clean up should be complete today.
0751725134 2014-11-05 Production Wells No Violations On the date of inspection I met with Gulfport personnel Brad Burwell, Hal Miller, and Nathan Ray. Gulfport was in the process of removing production tanks and hauling them off site. After the tanks are removed they will begin excavation around where the tank battery was located to determine how much soil was contaminated by the condensate leak. On the day of inspection, the open excavation would remain covered due to the rain forecast.
0905044370 2018-03-19 Preliminary Restoration No Violations On the day of my inpection I found the Lyon pad to have 3 properly labeled and producing plunger lift wells. This well has 100 psi on the surface casing and no pressure on the intermediate casing. Production equipment includes four 400 barrel steel tanks and one 50 barrel steel tank, all are contained in a steel dike. Three sand seperators, a heater treater, and a combustor are also onsite.
0990199669 2013-11-10 Production Wells No Violations Inspector Grimm and myself are responding to a fire which is burning on the Lyon pad. While covering a H2S complaint in Tusc.Co. with the Freeport Fire Dept which also had been called for a leak on # 34-157-2 3695 Tipka Operating - Willison well. We received a notification that there was a big fire on the Lyons Pad in Harrison Co. We left with the Fire dept and traveled to the Lyons pad. We arrived to find the flare stack on fire with flames falling over the top and burning down the flare tube and onto the ground in an area of 10x10 ft. The normal operation of the flare stack is to burn the gas from the vapor recovery system. This was a burning liquid, later found to be condensate which was not suppose to be in the gas system. The Freeport Fire Dept put out the fire on the flare stack and all of the production operation was shut in until the repairs to the flare stack could be made. Contact Deputy Chief Worstall and advise of the fire and the shut down of the operation.
1010661608 2014-11-03 Production Wells No Violations On the date of inspection I met with Kris Andersen, Dave Bockleman, and Dave Overstreet from ALL Consulting. Mr. Bockleman and Mr. Overstreet were on site for a brief time to survey the area around the well pad for possible locations to set monitoring wells. I was on site with Kris Andersen as the excavation of condensate contaminated soils commenced in a west-northwest direction. Kris Andersen had met with Mark West representatives earlier in the morning and had permission to enter their pipeline Right-of-Way to continue excavating until the end of contamination was found. While I was on site the excavation extended beyond the gravel access road to the west of the well pad. The excavation continued to find fractured zones in the weather bedrock that were found to contain condensate. The excavation will continue while Kris and Ryan Hegburg from Hull and Associates periodically take Photo Ionization Detector (PID) readings from potentially contaminated soil and gravel. While I was on site, there was personnel beginning to disassemble the equipment on and around the tanks so the tanks can be removed on Wednesday 11-5-14.
1031364674 2014-11-12 Production Wells No Violations On the date of inspection I spoke with Mike Dablemont from ALL Consulting and Dan Sansone from Hull and Associates. I was shown the open excavation by Mike Dablemont. They are continuing to excavate directly west of where the leak was observed. At the time of my inspection they had excavated as far west as the Mark West gravel access road. Dan Sansone periodically field screened soil with a Photo Ionization Detector (PID). It is visible from the photographs that I uploaded on the shared network drive that some areas are getting elevated PID readings from where Dan painted the ground. However, it is difficult to see a pattern to the contaminated areas. ALL Consulting and Hull and Associates will continue excavating and field screening samples with the PID. Dan Sansone also bagged and field screened samples from below where the tank battery previously stood. He created a 15 feet by 15 feet field screening grid across the tank battery area and detected no elevated PID readings.
1288109199 2014-09-30 Production Wells No Violations I received a call today from Kris Anderson and Brad Burwell of Gulfport Energy to advise me of a condensate spill outside of containment on the Lyons Pad. The spill occurred while the high pressure dump was operating and sending fluid to the production tanks. They noticed that the liner was lifting while the dump was operating and the leak was discovered at that time. It appears that 2 to 3 gallons of condensate made it out of the containment for the production tanks. When I arrived they were in the process of removing equipment so that the liner could be removed and the area below excavated. The plan is to excavate the contaminated soil back to the dump line and make repairs and replace with clean fill and repair the liner.
1382531354 2013-10-22 Production Wells No Violations I am on site today to check on the operation status of the well and the pad. I found that the well has proper identification and that it is in production. I found no violations on site at this time.
1388453112 2013-12-24 Production Wells No Violations I received a call from Kyle Baldwin who said he talked with Mrs. Rohal about a water problem caused from water running off of the Lyons Pad. He said she complained that the run off had a milky white color and also had a oil and gas odor. When I arrived today there was no water in her yard. There was no sedimentation in the yard. It has snowed over night and the ground in partly covered in snow. I checked the water in the culvert next to the road with a quantab and it was 80 PPM which is very good. There was no sheen on the surface and I did not detect any odor . Next I went below the yard around to the next road and went down the hill to see if I could find water with some sign of impact. I found clean clear water and did not witness anything in the water. I traveled over a 1/4 mile down grade where the water would run. I called Kyle Baldwin back and informed him of my findings. Kyle did not call me back.
1389212581 2014-01-06 Production Wells No Violations Today we are meeting with Harrison County Road and Highway Dept. and Blake Arthur and Hal Miller to review the water run off issue. The Highway Dept along with Blake Arthur from the Columbus Office ODNR checked the road along with the road surface and the tie in to the Lyons Pad. They did not find a problem with the road or the road surface of the Lyons Pad construction. Blake checked the property next to the pad for sedimentation or erosion and could not find a problem. The culvert is scheduled for repairs and the method of repair has been approved by the County Road and Highway Dept.
1389703335 2013-09-27 Production Wells No Violations I arrived at Miss Rohals residents and did an inspection inside and out and found no evidence of vibrations coming from well. Miss Rohal advised her house started to vibrate when a generator mounted on a truck at the wellsite across the rd approximately 500 feet away started up. She said something in the well also makes her house vibrate. I then went to wellsite and asked the crew if they were doing anything which would cause the rohal house to vibrate they said no. all they were doing was plumbing. They advised Miss Rohal complains everyday about something on site. At this time I found no violations . Case closed.
1391175879 2014-01-30 Production Wells No Violations I am on site today because there was a fire on the Clay Pad which was similar to the fire on this pad. Both fires involved the Flare Stacks and the control valves which failed due to ice. I am on site today to see if the lines are both the same or if they have made any changes. I did see a that the valves are somewhat different in sequence. I met with the pumper and he advised that everything is under control and that even with the -15 temps that they did not freeze up enough to stop production. I was contacted by Supervisor Rocky King and he said that there was a complaint from Stephanie Rohal indicating that there may be strange things happening on this site. I looked a the video and told Mr. King that they were using a Hot Box Heater because it was only 2 degrees out. They use them all the time to keep lines from freezing or to thaw them out if they do. Nothing strange about the use of a Hot Box. There was nothing on the video showing gas or vapor present during the time it was filmed. There was no odor while I was on site. I checked the well head area and the production area where the video was shot with my gas detector and had a --0 reading. I could see that the culvert had been replaced at the entrance to the pad as planned. Due to the snow cover I could not see much more detail. I contacted Hal Miller from Gulfport to meet me on site at 10 AM Friday to review my findings and to go over the culvert installation.
1391186588 2014-01-31 Production Wells No Violations I am on site today to respond to a complaint with a video attached. It indicates that there my be a strange operating practice and acrid air odor. I reviewed the video with the pumper, Hal Miller and Chris Anderson from Gulfport Energy and Hall Associates. We watched the video and found the strange practice reported in the video was a Hot Box being used to thaw a frozen valve. While watching the video which shows the compressors and separators plus most of the production operation we did not see gas or vapor leaking during the entire clip. I did check the area with a gas detector and did not have a reading over- 0-. I ask the pumper if there was or had been a spill or a release of any kind. He said there were no leaks to report. I have received calls from Chris Anderson in the past to report spills as small as a quart but have not had a call since the fire on the Flare Stack.

1412252194 2014-10-01 Production Wells No Violations I am on site today to clean up the 3 to 5 gallon condensate spill. The clean up will be done by hand because of the location to buried lines and also electrical lines. The spill was under the liner for the production tanks and the liner has to be cut open and removed. During the removal of the liner the area had to be vented due to vapor from the condensate under the liner. The area inside the dike has 3 to 4 ft. high dike walls and it would act like a contained space to trap the vapor. The digging had to be done by hand down below the line which broke and caused the spill. The depth is 4 ft. down and 3 ft. wide and then thru the dike wall 12 ft. to the left of the entrance. The dump line will be moved above ground after the repairs. The contaminated soil will be removed to a landfill and the liner rebuilt with a spray on Rino Liner type material.
1412342770 2014-10-03 Production Wells No Violations I arrived today and found that the spill is bigger than what was first discovered. It appears that the leak under the production tanks liner may have been leaking for days. The area being excavated is 20 ft. long and 8 to 10 ft. deep and 7 to 8 ft. wide. The contaminated soil is being contained in a roll off and then removed to the landfill. The cleanup will continue today digging around the production lines and below the dike sidewall. More production equipment will need to be removed so that the excavation can continue under the lined area where the standup Separator in located.
1412786790 2014-10-08 Production Wells No Violations The clean up of the condensate spill continues today along the left side of the dike toward the downhill side. The excavation is now down about 7 to 8 ft. deep and 12 ft. wide and 40 ft. long. More excavation will be done today to the long portion of the dike away from the production tanks. The wells remain shut during the clean up process.
1412865374 2014-10-09 Production Wells No Violations I am on site today to meet with Jace Marshall and Brad Burwell along with multiple construction employees to go over the amount of deconstruct which is needed on the production facility. The condensate spill which was discovered on Sept 30 is now much larger than it first appeared. It has gone from a 2 to 3 gallon spill to most likely in the hundreds of gallons. 26 roll off containers are full of contaminated material with more found Wedsday evening. It now will be necessary to bring in bigger equipment to remove dirt on a larger scale. The area where more equipment is needed will be the production tank area and side wall down to the toe. The dike side is 35 to 40 ft. deep and the width will be 20 ft. and 60 ft. long or more. Clean up will continue until all of the condensate has been captured and removed. I collected photos and forwarded them to Brent Bear and Blake Arthur. I also spoke to both about the details of the clean up and who should be updated.

We are on site to review the clean up of the condensate spill from 9/30/2014. I was able to walk thru the spill as to where we have excavated and what may need done. It appears that samples need to be taken along the sidewalls and bottom of the excavation to insure we have removed all of the contamination. Samples from the soil excavated off the top need to be taken to verify that the soil is clean and can be put back in place. The clean up is to continue thru the weekend.
1413465034 2014-10-15 Production Wells No Violations I am on site today with Kris Anderson of All and we are going over the progress of the clean up of the condensate spill which was discovered 9/30/2014. The area has had further de-construct on the production end of the pad. The line heaters and the production lines have been mapped and removed. They needed to be moved so larger equipment could be brought in to continue the condensate clean up. Thursday should be the day the equipment is in place to finish the clean up.
1413810038 2014-10-20 Production Wells No Violations Clean up of this facility continues today. The line heaters and separators have been removed so that the area below can be excavated and removed. The condensate clean up which began 9/30/2014 has been detected under the production equipment and down toward the toe of the pad. They have not started to dig toward the large standup separator at this time.
1413980655 2014-10-21 Production Wells No Violations I am meeting today with Kris Anderson and Brad Burwell to review the clean up of the condensate contamination. It is raining today and I arrived just during the covering of the excavation. All of the line heaters, compressors and vapor systems have been removed so that digging below them could continue. The main production line needs to be removed along with the header stands. The excavation is now 150 ft. long 50 ft. wide and 30 to 35 ft. deep. More digging to depth is to continue. Readings with a P.I.D meter have been used to determine the area of contamination. After the site was covered I met to discuss how the operation is to be rebuild and advise Rick Tripple be copied with the lab results.
1414504109 2014-10-27 Production Wells No Violations I am on site today to review the clean up of the condensate spill which began 09/30/2014. I am meeting with Brad Burwell and Kris Anderson along with Hull Consulting. The excavation has increased in depth length and width. I was advised that during the last portion of the excavation they encountered fractured sandstone that had water and condensate in the sample. I collected photos to send to Rick Tripple and Blake Arthur along with the rest of our staff to review as needed. Because of the depth of the excavation and the fractured material I contacted Rick Tripple to advise and to see what actions he would advise me to pursue. He wants to schedule a meeting to review Gulfports findings and to discuss the remediation plan or need to develop one after todays findings. Collect 13 photos today and merge them with earlier photos.
1414506096 2014-10-28 Production Wells No Violations I am on site today to meet with Kris Anderson and Brad Burwell. On Monday I collected photos of the area of excavation which had the fractured sandstone and water mixed with condensate. I went over those photos with Rick Tripple and he wants to schedule a meeting with Gulfport to discuss the remediation plan and to have more sample information forwarded to him. I advised Brad and Kris to send that information. I also advised they send sample information for the production tanks grid that had high condensate readings.
1415295322 2014-11-06 Production Wells No Violations Clean up of this site which began 9/30/2014 continues with the removal of the production tanks. The tanks and the dike around the tanks are being deconstructed. The berm and the plastic liner are also being removed. I met with Hal Miller and Kris Anderson and was advised that with the rain only the gravel is being dug up and removed today. The rains are on and off for today so the dike area is to be worked on and covered. The area on the west side of the location is covered and will remain covered until the rain moves out of the area.
1415550720 2014-11-09 Production Wells No Violations Met with Kris Anderson and Brad Burwell on site. The area where the production tanks and the dike were located are being excavated today. They are down 2 to 4 ft. below the gravel on the surface and are waiting on a PID meter to test for condensate to confirm clean readings. The dig to the North of the contaminated area appears to have clean readings and they plan to send samples to verify there findings. The area to the west along the gravel road appears to be reading clean and digging will continue to the south and east until clean readings are determined. The excavated area now is 275 long 150 ft. wife and 50 ft. deep. Work will continue until the next rain event forces a stop. Meetings are planned Monday and later in the week with ODNR.

Below where the production tanks were located is where digging continues with PID readings. The digging is to the toe of the hill and below. More digging will be done on the west side of the production area and toward the south until PID readings are clear.
1416492662 2014-11-20 Production Wells No Violations I am on site today with Brad Burwell and Kris Anderson to review the clean up which started on 9/30/2014. I was advised that the removal of the contaminated soil has concluded. After testing the excavated soils with a PID meter they feel they have removed all of the contamination. They have surveyed the excavation and included the location matched with the depth for 3-D ground water modeling. Next they will develop a grid and collect samples to verify that the contamination has been removed.
1430395600 2015-04-28 Production Wells No Violations During my inspection of the site I found that the tank battery has been moved from the left side of the entrance to the back right corner of the pad. There are 6 new 400 bbl. Tanks inside a new metal dike with a spray on coating acting as the seal for containment. All of the production equipment has been moved and re-organized to the opposite side of the pad in the back right corner. The site of the excavation has been backfilled and graded. Vegetation in this area needs to be established to keep erosion from occurring. As I continued down hill past the excavation site I found a monitor well in place on the left side of the gravel road.

I found that no work on filling the hole which was excavated to remove the contaminated soil has been started. The production tanks remain disconnected and no wells have been placed back into production.
1467306085 2016-06-30 Production Wells No Violations I received a call from Mr. Ron Ott of 27000 Friendly Ridge Rd to complain about the color of his water in the pond. He said that it has turned from a clear pool looking water to a brown stained water. He said that he thinks it is from this well site. I checked the pond for signs of contamination and did not find any fish kill, odor, sheen or dead vegetation. The water did appear brown in color. I collected photos of the pond banks or slope. The pond had been drained to complete repairs and while down is eroding along the slope of the exposed bank walls. There is gulley erosion around the entire pond. I returned a call after an inspection of the pond and the well site to Mr. Ott. I did mention the erosion to Mr. Ott and he said that the pond had changed colors 4 years ago. When I mentioned the erosion occurring on the pond sides he said that the color change occurred prior to the erosion. I contacted Region Supervisor Brent Bear and Hydro Geologist Jake Glascock to advise of my finding. I contacted Rick Tripple to obtain data from the monitor well on the Lyons pad.
1760207106 2014-10-30 Production Wells No Violations On the date of inspection I was on site to check the status of the condensate spill clean up. I spoke with Kris Andersen from ALL Consulting and Brad Burwell from Gulfport Energy Corp. At the time of my arrival the crew was covering up the excavation due to possible rain, so I was unable to visually survey the open excavation. I was informed by Kris Andersen that the crew had excavated to the north and to the west since ODNR Inspector Mark Moreland had last been to the site. Kris believes that they had reached the end of contaminated soil on the north side. Kris stated that they were still getting elevated Photo Ionization Detector (PID) readings while excavating to the west. At this point, they are encroaching on the Mark West Right-of-Way to the west and will need further utility location and permission from Mark West to continue excavating contaminated soils to the west (toward the ravine and gravel road). Kris informed me that over the next three days they will be disassembling the tank battery and its components. A crane will be available on Monday 11-3-14 to remove the tanks. Kris will begin by removing the tanks and liners and excavating 2 feet of gravel and soil to begin assessing that portion of the well pad. Kris informed me that they had sent two samples to a lab for analysis. One sample was of contaminated soil for them to get a fingerprint of what compounds are present in the condensate. The second sample was of clean soil from the north end just below where most of the pipe was buried. Kris also informed me that letters would be mailed to landowners within a day or two in order to re-sample all of the locations that were sampled during the pre-drill baseline sampling event.

2015079938 2014-11-17 Production Wells No Violations On the date of inspection I met with Kris Andersen with ALL Consulting. At the time of my inspection, there was no work being done within the open excavation due to weather conditions. Two excavators were in the process of loading contaminated soil into trucks to dispose of off site. Kris informed me that he believed they had excavated to the end of contamination at the end of last week. He plans to have the contractors excavate the rest of the contaminated soil that was used to work from as they work their way out of the excavation. He plans for Hull and Associates to sample in a grid across the excavation later this week. ALL Consulting is still in the process of resampling water wells that were sampling during the Pre-Drill Survey. The crew is having difficulties making contact with various landowners.
