1189190551 2016-02-16 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. All wells on location are using the plunger lift system as the method of production. RBDMS production records indicate that the wells are in production.
The production equipment on the pad consists of the following:

10 400 bbl. Production tanks 3 Producing wells 3 Sand separators

1 Low pressure separator tower/Vapor recovery system unit. 1 Combustor 1 Horizontal heater-treater. Equipment present on the location that is not associated with the production equipment consists of: 3 18 Wheelers with trailers 1 track hoe 1 Short bed semi 1 dump truck 1 skid steer 1 power plant 2 pickup trucks

8 small trailers 1 40 foot box trailer 1 16 foot box trailer 3 fuel tanks 35 joints of 9 5/8 inch diameter casing. There was approximately 4 to 5 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1329762510 2013-10-03 Preliminary Restoration No Violations On the day of my inspection the site passed preliminary restoration according to 1509.072A. Made phone call to Matt Amos Drilling Foreman of Chesapeake Energy Corp and he referred me to John Snook Drilling Superintendent requesting that the cellars be pumped out on the Gotshall Pad and he confirmed that it would be done today.
1346849983 2012-08-27 Drill / Deepen / Reopen No Violations Arrived on location for cementing of surface pipe , 13 3/8 pipe set at 615 with 530 sks of class a cement, covered job for mark moreland. He advised he wants to fill out all cement tickets when well is completed.
1353934782 2012-08-22 Drill / Deepen / Reopen No Violations Set 100 ft of 20 inch conductor pipe and cemented with 265 sks - O-Tex Cementing Service. 26 inch hole. Set Tin Horn and cemented in place along with the conductor pipe. Had enough return on the cement to make a goog seal between Tin Horn and 20 inch casing.
