1358169053 2012-01-13 Drill / Deepen / Reopen No Violations I am on site today because the Auger Drill rig has encountered a problem. During the drilling of the 36 inch conductor hole they hit a rubble pile from the old coal mine. The hole has colapsed and we are cementing it back. Halliburton is on site and could only get cement back to 25 ft from surface. The Auger Drilling rig plans to drill out the cement in the morning after repairs to the rig have been completed. After drilling out the cement the conductor and the Tin Horn will be set and cemented to surface.
1358447676 2013-01-17 Plug / Plug Back No Violations I am on site today to witness the plugging of the 36 inch conductor hole. During the installation of the 30 inch diameter casing it broke apart. The operator wants to leave the conductor in the well and cement it rather than fish it out. The hole has been drilled so crooked that casing can not be installed so they will cement it and skid the location. I have contacted Steve Opritza and Rocky King to advise of the lost hole. Halliburton is to plug the well. W/O cement trucks to arrive.
1359049423 2013-01-20 Drill / Deepen / Reopen No Violations I arrived on this location to witness the cementing of the conductor pipe. The borehole was drilled crooked which caused the 30 inch conductor pipe to break.
1359053591 2013-01-19 Plug / Plug Back No Violations I arrived on this location to witness the grouting of the conductor hole with ready-mix. 20 tons of fireclay have been placed in borehole to plug formation so cement will seal off and come to surface. Tarr redi-mix grouted well with 50 yards of 9 sack grout and did get cement to surface. I was on site from 7:30am until 10:30 am at which time I had to leave because I was out of hours. Job was completed at noon. Inspector Moreland will pick up cement tickets on 01/23/0213.

1931946084 2013-01-18 Drill / Deepen / Reopen No Violations I arrived on this location to witness the plugging of the conductor hole which is 100 foot deep. The borehole was drilled crooked which made the 30 inch conductor pipe part. The crew cant get the pipe out so the well has to be plugged. Halliburton pumped 825 sacks of class a cement on this date and the cement would only come up 50 foot. Crew is going to put 20 tons of fire clay in to borehole then grout with readymix at a 9sack grout on 01/19/2013.
2094760279 2018-10-30 Final Restoration No Violations On the day of my inspection I found this pad has transferred to Ascent resources and does have a well on it. Permit number is 3406721128 and is labeled. Previous inspection reports indicate that this well (1083) was drilled with a conductor rig and plugged back to surface with the drilling permit. My inspection revealed no well at this location. Molly Regan from Hess called me on 10/29 on advisement from Cynthia Marshall. Hess has sold their holding to Ascent and this permit did not transfer because it has been plugged and needs a final restoration. A final restoration can be granted as there is another permitted well on the site.
