I contacted Supervisor Gene Chini and set up a meeting with Wayne Schalk and Gene Chini to review problems with the pad.

Owner Operator shall plug mouse hole in accordance with OAC 1501:9-11-11(A).

1510363026 2012-08-07 Drill / Deepen / Reopen No Violations Met on site today with Supervisor Gene Chini and Mines Supervisor Wayne Schalk. Hess Ohio had a core rig on site and we did collect at least 4 samples around the conductor. Supervisor Wayne Schalk advised that a 15 ft deep diversion ditch be cut along the uphill side of the location. A french drain type system would need to follow the cut and carry the water around the location to prevent it from running down along side of the conductor hole.
1513284839 2017-12-14 Production Wells No Violations On the day of my inspection I found this well pad containing only one wellhead. Well is equipped with christmas tree style wellhead. No production equipment onsite. Pad has weeds growing through the crushed limestone covering. This inspection is intended to review the well as Hess is applying to put this well into temporary inactive status for the third year. Production casing has 4500 psi on it, two other unlabeled valves read 50 and 0 psi. Valve coming from cellar reads 50 psi and valve on tubing hanger reads 0.
1540989141 2018-10-31 Temporary Inactive Well Status No Violations On the day of my inspection I found this pad to contain one well with no production equipment. Well is in temporary inactive status. Area resident James Kerr 740-942-3196 had called me yesterday with questions about this well, taking gas without a lease, and fracking. Ascent has recently purchased this well from Hess. Hess fracked the well in 2013. Lateral was drilled to the northwest. Mr. Kerr lives a little over 2 miles west of pad site and lateral is no where near his property. I called him back today to share this location information and discussed the process of drilling and stimulating a well.
1547945291 2016-02-09 Production Wells No Violations I arrived on location accompanied by inspector Mark Moreland to conduct a follow up inspection on a Compliance Notice 684435865. On the day of my inspection I witnessed the debris on the location has been removed. However proper identification has not been installed on the well head and with the weather conditions it could not be determined if the mouse hole has been plugged. Another inspection will have to be conducted to check on the status of the mouse hole. The Compliance Notice cannot be terminated until all issues are resolved.
