0196442315 2018-07-12 Completion Testing No Violations On the day of my inspection I found Stone rig 15 on location to drill out frac plugs. Current surface annular pressure is 0 and intermediate annular pressure is 30 psi. Currently drilling out on permit 1542. South and east of the wellheads some crystalization was noted on the pad. David Reese and Blake Riley are company men for Chesapeake. They were informed of the potential containment issue and said that would get it fixed and cleaned up right away. The area is dry and does not appear to have been a large volume of fluid that escaped primary containment.
0202532992 2018-07-24 Completion Testing No Violations On the day of my inspection I witnessed Stone rig #15 in onsite. This well has had all plugs drilled out. Surface annular pressure is 0, intermediate annular pressure is 50, and production casing pressure is 2200.
0278095028 2018-08-23 Production Wells No Violations Well has been drilled, stimulated, and drilled out. Well has not been flowed back yet. ID myself to facilities foreman Trevor Joilet. Current activity on pad is production equipment construction and installation. Drilling of 4 new wells on this pad started in december of 2017. They have not been flowed back yet. Call to Chris Huey production foreman stated that flowback should start on 8-30 for the 4 new wells and he hopes to bring the 5 existing wells back online around mid September. Production equipment includes four line heaters, 4 sand seperators, and 4 GPUs for the new wells. Existing wells have 5 sand seperators, 5 GPUs, 2 combustors, 3 compressors, and 7 400 barrel steel tanks in their own containment. All nine wells on the pad are shut in. Surface annular pressure is 0 and intermediate annular pressure is 70.
0975569262 2018-02-06 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 571 drilling an 8.5 inch diameter hole at 8268 feet. They started drilling their cruve at 7776 feet and plan to land it around 9000 feet. Pad drains are shut and containment is functioning properly. No gas flare during my inspection.

1070605444 2018-01-30 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Patterson 571 drilled a 17.5 inch diameter open hole to a depth of 682 feet. Previous casing set and cemented was 20 inch diameter conductor casing to a depth of 120 feet. Wyoming Casing ran 672 feet of 13.375 inch diameter casing, j-55, 54.5 lbs/foot. Casing was centralized with 15 centralizers. Shoe track is 42 feet long. Schlumberger was onsite with 662 sacks of class A cemcrete tail cement. They also had 95 sacks of class A neat scavenger cement. Cement weighed with mudscale at 15.6 ppg for scavenger and 17.0 ppg tail cement. Yields of 1.18 and 1.08 respectively. 50% excess ran in open hole. Cement displaced with 95 barrels of fresh water. 25 barrels of gel and 10 barrels of water pumped ahead of cement. 20 barrels of scavenger cement and 33 barrels of tail cement returned to surface, and floats held with 0.5 barrels back. Cement to surface with 20 barrels left in displacement weighed 16.5 ppg.
1134806764 2018-01-29 Drill / Deepen / Reopen No Violations On the day of my inspection I found Patterson 571 rigging up to spud this well. Company man Will Doby anticipates a cementing the surface casing tomorrow. Walk around location found a small volume of diesel fuel (<2 gallons) was spilled on containment. They were instructed to get it cleaned up and to figure out where it came from. No holes notes in containment and is functioning properly.
1433858777 2017-12-07 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augured a 26 inch diameter hole to 120 feet. 120 feet of 20 inch diameter conductor casing was run. Express was onsite with 230 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 50% percent excess ran. Cement was displaced with 39 barrels of fresh water. Cement returns 5 barrel to surface then fell back. Phone call from Chris Schmalz on 12-9 stated it fell back to 75. He was told to top out with pump truck and not grout truck. Top out was not witnessed, but ticket says they pumped 262 sacks.
1588752166 2018-07-02 Completion Testing No Violations On the day of my inspection I found a Halliburton frac crew rigged up to this as well, as well as permits 21541 and 21540. They are currently pumping on stage 80 of 93. stage is from 11222-11102 and was perforated with 5 guns, 6 shots per gun, on 30 foot spacing. 330000 pounds of sand will be pumped this stage.
1632558883 2016-02-24 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well 34067213780000 34067213790000 34067213800000 34067213810000 and 34067210570000 are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order. Permitted well number 34067210670000 has not been drilled.
The production equipment on the pad consists of the following:

6 400 bbl. Production tanks 5 Producing wells 5 Sand separators

1 Horizontal heater-treater 1 combustor 1 low pressure separator/ Vapor recovery system 1 compressor The site passed preliminary restoration in accordance with ORC 1509.072 A.

1848698558 2018-02-14 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Patterson 571 had reached their total depth of 23168. Wyoming casing ran 19000 feet of 5.5 inch diameter production casing and could not get any further. All the casing was laid down. The rig is tripping back in to ream the tight spots in the lateral section.
1959174472 2018-05-14 Completion Testing No Violations On the day of my inspection I found halliburton rigged up to the 6H, 206H, 10H, and the 210H. This well has had 3 out of 93 stages fracked. The 9.625 inch diameter annular pressure is 217 psi. The 13.375 inch diameter annular pressure is zero. Frack is currently shut down due to lighting strikes within 10 miles.
2083313714 2018-07-18 Completion Testing No Violations On the day of my inspection I witnessed Stone rig 15 was tripping back in the hole after changing bits. They have drilled 82 out of 94 plugs in the wellbore. The 83rd plug is at 21280 feet. Surface annular pressure is 0 and intermediate annular pressure is 0. Containment has been repaired from last weeks inspection and contaminated gravel was removed.
