1347036266 | 2012-09-07 | Production Wells | No Violations | At 7 :10 A.M. I received a call from Bob Adams of Chesapeake about a fire at this well site. He advised that a fire had occurred at 2:00 AM and that by 3:00 AM it was out. He also advised that two workers had been injured. One worker had burns to both hands and was life flighted. They other worker fell over something on the ground and refused treatment. I contacted Supervisor Hill and Chini to advise of the fire and traveled to this site with Inspector Grimm. We found that the fire was in a contained area about 65 ft x 125 ft. Inside this contained area were 4- 300 barrel tanks.The tanks were in place to accept flow back water from the new well. When one of the tanks became full a 3 inch water pump was used to move water from the full tank to another tank along side. During the tranfer of water from the full tank to the secondary tank. The gasoline engine on the water pump ignited vapors from the flow back fluid causing a minor explosion and a fire. The two tanks with flow back fluid cooked off but did not launch or burst. Nothing leaked outside of the tanks and no spill occurred. One of the workers on site,{ Allen Carter} did receive burns to both hands. He was Life Flighted for treatment. The well was shut in during the fire and remains closed. No further testing or operations will occur on this site until a review of how the fire started has been conducted. Updated information has been sent to Uniontown office and Chief Simmers. |
