0151514659 2012-07-12 Drill / Deepen / Reopen No Violations I am on site today to check the rig status after cementing the intermediate 9 5/8 casing string the night before. The Weatherford well head needed to be repaired and has just been completed. Drilling is now set to begin.
0368609613 2016-02-22 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells 34067210640000 and 34067210630000 are utilizing a plunger lift system. RBDMS production records indicate that the wells are in production. Permitted wells 34067210770000 and 34067210840000 have not been drilled, but the tin horns have been set.
The production equipment on the pad consists of the following:

8 400 bbl. Production tanks 2 Producing wells 2 Sand separators

2 Horizontal heater-treaters 1 combustor 1 compressor 1 Low pressure separator tower/Vapor recovery system unit. The site passed preliminary restoration in accordance with ORC 1509.072 A.

0469945303 2012-07-10 Drill / Deepen / Reopen No Violations I am on site today with Inspector Farnsworth. Patterson Drilling rig # 259 is setting the intermediate string of 9 5/8 casing. Schlumberger is cementing 2662 ft of 9 5/8 casing with 984 sks. The cement returns were 20 bbls or more. Inspector Farnsworth is to complete the cementing report. Note on job - The Weatherford head was constructed wrong and caused several hours of delay before cementing could occur.
0920872202 2012-07-23 Drill / Deepen / Reopen No Violations I am on site today to witness the running and cementing of the production casing. A valve broke on the Cement Manifold and fell 40 ft to the rig floor. Nobody was on the floor at this time and it did not result in an injury. During this event oil base mud was pumped 80 ft out from the manifold and onto Schlumgergers cement crew and equipment. The area was on a contained section and the clean up was completed while I was on site. See Cement Ticket for cement information.
1158154417 2017-10-12 Production Wells No Violations On the day of my inspection I found the pad to contain two plunger lift wells and a conductor pipe inside of a cellar. Production equipment includes six 400 barrel production tanks, two vertical seperators, two GPUs, one combustor and a vapor recovery system. This well is labeled as 201069 onsite but this is incorrect. The Henderson south pad nearby is also has a well labeled 201069, which is correct.
1309336230 2012-07-16 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling operation. I found Patterson Rig # 259 Drilling ahead at 6625ft on a 8 3/4 hole. They are going to circulate bottoms up and then make a bit trip, then reduce down to a 8 1/2 inch bit. They are also changing over to fluid and will continue fluid drilling to complete the total depth. I was advised that the length of drilling time remaining would be about one week.
1347036266 2012-09-07 Production Wells No Violations At 7 :10 A.M. I received a call from Bob Adams of Chesapeake about a fire at this well site. He advised that a fire had occurred at 2:00 AM and that by 3:00 AM it was out. He also advised that two workers had been injured. One worker had burns to both hands and was life flighted. They other worker fell over something on the ground and refused treatment. I contacted Supervisor Hill and Chini to advise of the fire and traveled to this site with Inspector Grimm. We found that the fire was in a contained area about 65 ft x 125 ft. Inside this contained area were 4- 300 barrel tanks.The tanks were in place to accept flow back water from the new well. When one of the tanks became full a 3 inch water pump was used to move water from the full tank to another tank along side. During the tranfer of water from the full tank to the secondary tank. The gasoline engine on the water pump ignited vapors from the flow back fluid causing a minor explosion and a fire. The two tanks with flow back fluid cooked off but did not launch or burst. Nothing leaked outside of the tanks and no spill occurred. One of the workers on site,{ Allen Carter} did receive burns to both hands. He was Life Flighted for treatment. The well was shut in during the fire and remains closed. No further testing or operations will occur on this site until a review of how the fire started has been conducted. Updated information has been sent to Uniontown office and Chief Simmers.
