0203635236 2015-11-10 Production Wells No Violations With my inspection of this facility, I found no signs of prior impact from previous spills. The area where the cuttings had been originally stored, then excavated and removed , now has good vegetation cover.
0535357317 2017-09-06 Production Wells No Violations On the day of my inspection I found the Wagner wellpad to contain three properly labeled texas style wellheads. No pressure on surface casing and no gauge noted for intermediate casing. Cellar is full of water and has occasional bubbling. Also appeears to be some oil scum on top of the water. Production equipment includes four horizontal seperators, four 400 barrel steel tanks, a 50 barrel steel brine tank, and a combustor. Production records current in RBDMS.

1364498381 2013-03-28 Production Wells No Violations I received a call from Gulfport Energy Production Supervisor to inform me of a fire which had just occurred on their Wagner Pad in Harrison Co. The call came in around 8 :45 am. Rick advised me a drip tank used to blow drip gas from the separator dump and the drip from the vapor recovery system had been on fire. When I arrived I found that the fire had been put out with foam by the Cadiz Fire Department. The drip tank is used to blow moisture from the separator after it dumps into the production tanks and from the vapor recovery system. During that blow down process a cloud of vapor formed and traveled 25 to 30 ft. and was ignited by a compressor operating the vapor recovery system. The cloud of vapor burned back to the 50 barrel tank and it ignited. The fire burned above the tank and the tank did not rupture. There were no spills or leaks so no clean up was needed. Nobody was injured during the fire.
1398951111 2013-04-01 Production Wells No Violations I am on site today with Deputy Chief Bob Worstall and Supervisor Rocky King to review the drip tank fire. When we conducted our inspection I found that the 50 bbl drip tank has been deleted from the system. The vapor recovery system is passed through the flare and any remaining moisture is placed in the production tank.
1530697151 2012-07-11 Completion Testing No Violations DNR Inspector Mark Moreland and I were on site to check the status of the well and check the other activity on the well site. Our inspection revealed that the well has been fracked and is shut in at the current time. Our inspection also revealed that a crew is constructing the containment area for the storage facility and another crew is running the gas lines for this well. No violations were noted during the inspection of this well site.
