0084327255 2016-08-29 Production Wells No Violations On the day of my inspection I witnessed the well is utilizing a pump-jack for means of production. The well bore consist of 8.625 inch diameter surface casing, 4.5 inch diameter production casing, 2.375 inch diameter production tubing and pump assembly components. A prime mover is in place and the well was pumping at the time of my inspection. Production equipment for the well bore consists of a 210 barrel production tank, a 30 barrel brine tank and a horizontal separator with a 2 inch diameter meter run. The meter chart indicated the well was in producing order. All production equipment is placed in a dike that measures approximately 40*50 feet at an average height of 3 foot around the perimeter. Also placed in the dike is the production equipment for permitted well number 3406720969. RBDMS records indicate the well is in production and no violations were noticed with the conduction of my inspection.

-up process was in place. Atlas used the mini excavator to build a dike around the area of the spill and dig a 3 foot deep by 2 foot wide by 5 foot long pit to contain the oil. After witnessing the spill I then place a call back to Terry White to conform the information Chris had given me. Terry stated that Atlas would need to provide a manifest sheet and to use lime/ fertilizer after the clean-up process was completed to neutralize the impacted area. After speaking to Terry I then forwarded the information to Chris. While on site I witnessed the clean-up process which included, a tanker truck used to vacuum the oil from the pit and then placed the oil inside a production tank that was already present on location, Atlas employees steam cleaning all contaminated production equipment and the use of a d

1453825330 2016-01-25 Production Wells No Violations On the day of my inspection following up on a release that occurred 1/25/2016, I witnessed the remediation of the spill has been fully accessed. I could not see any visual signs of pollution or contamination. All production equipment was cleaned and peat-sorb was placed in the areas affected by the spill after the clean-up was conducted.
