0203622287 2016-02-10 Production Wells No Violations On the day of my inspection, I witnessed the well head is equipped with a production tree that is tied into a flow line going to the production equipment. Production equipment for the well bore consisted of a line heater, sand trap and a gas production unit. Production equipment that was shared with permitted well numbers 34067213780000, 34067213790000, 3406721380000 and 34067213810000 included one gas compressor, a vapor recovery system, a gas combustor and 6 400bbl. production tanks. While I was on site I performed a visual inspection, were I found no signs of pollution or contamination.
0343873693 2014-01-17 Preliminary Restoration No Violations On the day of my inspection the site passed preliminary restoration according to 1509.072A.
0400252182 2016-02-24 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted well 34067213780000 34067213790000 34067213800000 34067213810000 and 34067210570000 are producing off of natural reservoir pressure. RBDMS production records indicate that the well is in producing order. Permitted well number 34067210670000 has not been drilled.
The production equipment on the pad consists of the following:

6 400 bbl. Production tanks 5 Producing wells 5 Sand separators

1 Horizontal heater-treater 1 combustor 1 low pressure separator/ Vapor recovery system 1 compressor The site passed preliminary restoration in accordance with ORC 1509.072 A.

0575373905 2017-09-22 Production Wells No Violations On the day of my inspection I found the Buell pad to contain five properly labeled wellheads. Wells are equipped with chritmas tree wellheads. Gauges show no surface or intermediate casing pressure. Production equipment on the pad includes five vertical seperators with gas production units, one heater treater, four line heaters, three 400 barrel steel condensate tanks, three 400 barrel steel produced water tanks,a compressor, vapor recovery system and a combustor. Tanks are in their own steel containment. Production records current in RBDMS.
1130930142 2010-12-08 Drill / Deepen / Reopen No Violations Inspection revealed that Nomac Drilling is moving on location and starting to rig up. They advised that the derrick should be on site tomorrow and should be finished rigging up sometime on Friday. They are hoping to be spudded late Friday night sometime. No violations at this time.
1535037705 2018-08-23 Production Wells No Violations On the day of my inspection I was checking on the well for idle well complaint number 5938. Well is currently shut in for SIMOPS. ID myself to facilities foreman Trevor Joilet. Current activity on pad is production equipment construction and installation. Drilling of 4 new wells on this pad started in december of 2017. The new wells have been stimulated and plugs drilled out. They have not been flowed back yet. Call to Chris Huey production foreman stated that flowback should start on 8-30 and he hopes to bring the 5 existing wells back online around mid september. I contacted Bobbye Grubich (330-325-1890) and informed her of the status. Complaint can be closed. This well does not appear to have a surface annular gauge and intermediate annualr pressure is 0.

1827662996 2012-07-11 Preliminary Restoration No Violations DNR Inspector Mark Moreland and I were on site to conduct a preliminary restoration inspection of this production well. Our inspection revealed that the affected areas, not being utilized for the production of the well, have been successfully revegetated and there is no substantial erosion and sedimentation noted during this inspection. The Preliminary Restoration can be approved at this time. Note: Under restoration comments Pits Filled was not checked because there were no earthen pits used during the drilling and fracking of this well.
