0123528797 2016-04-14 Production Wells No Violations On the day of my inspection accompanied by inspector Mark Moreland, I witnessed that the tank battery consisted of a 210bbl tank, 50bbl brine tank, a horizontal separator and sales meter with a chart that dated back to the year 2015. The line pressure and the differential pressure on the chart are at zero psi. This indicates that the well is not in production. The well was equipped with 4.5 inch diameter production casing, 1.5 inch diameter production tubing and a plunger lift system. A patch was placed on the 1.5 inch diameter production tubing directly above the slips on the well head and gas could be heard coming from the patch at the time of my inspection.

1392048989 2014-02-10 Production Wells No Violations On the day of my inspection the ground was snow covered and I witnessed that the tank battery consisted of a 210bbl tank, 50bbl brine tank, a horizontal separator, a sales meter with a 1 inch orifice, all valves open, and a leak was found on the sales line with a current chart dated 2/4/14. The lease road was washed out and gated. The well was equipped with 4.5 inch casing, 1.5 inch tubing, a plunger lift system logically controlled with the valve open on the tubing and the flow line.
1528999605 2018-06-14 Production Wells Violations Noted On the day of my inspection I am following up on complaint 5434. Landowner originally complained about the well not producing and it is leaking gas. Emergency Operations Jason Blocker accompanied me during this inspection. Well should be shut in due to the Temporary Restraining Order issued by Trumbull County Court of Common Pleas, Chiefs Order 2015-535, Chiefs Order #2016-105, Chiefs Order 2016-172, and Chiefs Order 2014-43.

1726763810 2016-09-21 Production Wells No Violations On the day of my inspection following up on complaint number 5434 which was a complaint by landowners Rick and Judy Luyster who were concerned about the well not being in production and not providing domestic gas. Rick and Judy stated that they would just like to see the well plugged. Prior to my inspection I met with Rick and Judy. They informed me that the gas to the house was shut off years ago when the gas turned sour and declared unsafe for domestic use. With the conduction of my inspection I witnessed the tank battery consisted of a 210bbl tank, 50bbl brine tank, a horizontal separator and sales meter with a chart that dated back to the year 2015. The line pressure and the differential pressure on the chart are at zero psi. This indicates that the well is not in production. The well was equipped with 4.5 inch diameter production casing, 1.5 inch diameter production tubing and a plunger lift system. A patch was placed on the 1.5 inch diameter production tubing directly above the slips on the well head and gas could be heard coming from the patch at the time of my inspection. I informed Rick I would speak to my supervisor about the well and see what can be done to help resolve the issues they are facing.

-43, I witnessed that the production equipment included a 210bbl tank, 50bbl brine tank, a horizontal separator, a 2 inch sales meter with a 1 inch orifice and chart recorder with all valves open on the sales line. The well was equipped with 4.5 inch casing, 1.5 inch tubing, a plunger lift system logically controlled with the valve open on the tubing and the flow line. Identification is present and production records are current through 2012.

1978004199 2018-06-18 Production Wells No Violations On the day of my inspection I was accompanied by Inspector Pat Shrieve. We viewed the well and production equipment. Valves on well and seperator are open, need to return and check whether motor valve is open. 210 barrel steel tank is estimated to be half full. Call was placed to Big Sky on 6/20/18 and I asked if he could get the leak fixed. He said that he is not allowed to operate the wells but would try and get someone to fix it.

-working logic box at the well. The tank site consists of [1]210-barrel steel tank, [1] 1100-gallon plastic tank and a horizontal separator. The valves are open and are sweating down- stream from the 1- inch orifice plate. The chart has not been changed in some time and the wind- up clock may not be working in the chart meter. The production equipment is enclosed within an earthen impoundment. The identification is not legible but is posted on the tank.
