The entire area has been cleaned up and the contamination removed. The area has been graded and seeded with mulch for cover. The tank which was damaged has been replaced and the well is back in operation. |
1407003674 | 2014-07-31 | Production Wells | No Violations | I received a call from Bruce Levengood to inform me that lightning had struck a 210 bbl tank. The tank launched down range over 100 yards and resulted in a fire at the tank pad and surrounding dike area. The fire was extinguished by 6 responding Fire Dept. Cadiz- Scio - Jewett- Bethel- Bowerston- Tappan. The tank was in a dense wooded area and could have resulted in a forest fire. The fire was put out and most of the oil burnt up during the fire. An area near the production dike has some oil remaining which has been covered with peat sorb. The peat sorb has kept the oil from running off site but will still need excavated and removed. The amount which needs removed is in the 12 to 14 cubic yard amount. Pictures have been collected to show the area affected and the amount which remains to be cleaned up. The truck hauling the equipment broke down in route so it took longer for the clean up to begin. |

1589130478 | 2017-03-10 | Production Wells | No Violations | I went to this location to evaluate an oil spill. I found that the flowline had ruptured between the well and the tanks and < 5 bbls of oil had been leaked on to the ground in a marshy area next to a creek. Some oil got into the creek. The operator had a contractor there and they had put down some booms to contain the spill but some was leaking thru the ground into the creek. More booms were put into the creek to keep the oil from migrating downstream. I left instructions for a siphon dam to be built just N of the lease road and for a boom or booms to be placed way down stream as a failsafe. There are several booms in the creek N of the lease road. A vacuum truck was on the scene to try and remove the oil on the ground after the lease road could be made passable by the dozer on location. A backhoe was on the way to the site. OEPA (Trevor Erwin) was on the way from Columbus, ETA 5 PM to take over the clean-up. I spoke with Eric Bear, Wildlife Supervisor, and assured him that there were no fish or wildlfe in stress or deceased. I spoke with Randy Akins, Inspector and apprised him of the situation and he was to check the scene out on Saturday morning. I also reported the above information to Terry Whitehair, Regional Emergency Response Manager. |

The fire damaged the trees around the tank battery and you can still see how high up the on trees the fire was. The well is producing into a 210 bbl. Tank and using a 110 bbl. tank for brine storage. |
