0056689613 2016-10-11 Plug / Plug Back No Violations I am on site today to witness the plugging of the well bore. Nicks Well Plugging Service is rigged up over the hole and will be preforming all service work. Specialized Safety Services is on site and will be providing safety assistance if needed. When I arrived on location Chuck Woods (an Atlas Noble Supervisor) informed me that on Columbus Day that NWP was able to pull all rods and was ready to set the bottom plug this morning. Prior to working on the well bore Specialized Safety Services evaluated the well for H2S and found alarming levels of the gas with upward readings of 4 percent concentration (40,000 ppm). After the test NWP then treated the well once again with H2S blocker. After treating the well NWP then went to pick up the tubing and discovered that the tubing was hung in the hole. In order to free the tubing NWP displaced 20 bbl. of fresh water on the backside of the tubing. After the fresh water was displaced NWP was then able to free the tubing and set the bottom plug from 5450- 5050 with 30 sacks of Class A cement and displacing with 19.5 barrels of fresh water. After setting the plug the well was shut in and left over night.
0332302552 2017-05-23 Final Restoration No Violations On the day of my inspection I found this well to be plugged. Area needs vegetation to pass final restoration. Spoke with Chris from Atlas and he is going to have a crew reseed it.
0349184707 2016-10-13 Plug / Plug Back No Violations I arrived on location today to witness the reminder of the plug job. As stated in my previous inspection Nicks Well Plugging Service is rigged up over the hole and will be preforming all service work. Specialized Safety Services is on site and will be providing safety assistance if needed. Upon arrival I witnessed NWP in the process of pulling out the production tubing. After pulling out the production tubing, I then witnessed NWP ripping the 4.5 inch diameter production casing at a depth of 1315 feet. The pipe was ripped at a higher depth due to the H2S factors of the well and safety concerns. After ripping the pipe NWP then pulled the 4.5 inch diameter production casing to a depth of 1200 feet and set a plug from 1200-980 feet, using 67 sacks of Class A cement and displacing with 15.6 barrels of fresh water. Then once that plug was displaced, NWP pulled the casing to a depth of 200 feet and circulated cement to surface. I then arrived later that day to check on the status of the plug and found that the surface plug had not dropped. I then witnessed Atlas Noble cutting the surface casing 3 feet below ground level and plating the well bore before filling the hole.

1367648327 2016-10-07 Plug / Plug Back No Violations On the day of my inspection I met with Atlas Nobles Production Supervisor Chuck Woods to perform a site overview on the well, which is set to be plugged on the next work day. While on site Chuck informed me that he had been encountering some alarming H2S readings on the well bore. Chuck stated that he has treated the well once with 700 gallons of fresh water to 40 gallons of H2S blocker on the backside of the production casing and inside the production casing. He also informed me that plans to treat the well once again before preforming work on the well bore. I will be back on site after the weekend to witness the plugging of the well bore.

1576871904 2016-10-24 Final Restoration No Violations I am on site today to check the status of the plug job that was just recently completed. With the conduction of my inspection I did not smell or notice any indication of H2S. The location has been graded but vegetation has yet to be established.

However vegetation was not established due to the season. With the inspection of the well bore I did not notice any signs of plug malfunction. I did not notice any indications of hydrogen sulfide while on site.
