0160591092 2013-11-01 Plug / Plug Back No Violations I was on location at 7:20am with Inspector Mark Moreland and Nicks Well Plugging to witness and train in plugging a well that is producing hydrogen sulfide (H2S) from the Newburg Porosity Zone. OBriens Safety Services were on location to monitor the levels of H2S. The wellhead was in a converted wetland and everytime the service rig tried to raise the mast it began sinking. Nicks decided they would need to bring in a mudboat and large bulldozer in order to properly spot the rig. They would not be able to get the equipment and rig spotted until later in the afternoon and Inspector Moreland decided that plugging would not start until Monday 11/4/13. I left location and will return next week. No violations.

1383654051 2013-11-05 Plug / Plug Back No Violations I am on site today to witness the plugging and cementing operation. Nicks Well Plugging Service is ready to set the bottom plug thru 2-3/8 inch tubing. They pumped tubing volume to make sure it was clear and then set the bottom plug. Because of the H2S gas we did not circulate on the bottom plug and the Newburg plug which was set at 4800 to 4600 ft. Since we had reason to believe that the 4-1/2 inch diameter casing was parted or had a hole in it, the Newburg plug was necessary. Next plug was set after we established circulation. During the circulation the H2S level went up to 17 PPM and then after only a few barrels dropped off to 0 PPM which is what I would have wanted to verify my Newburg plug was in place and holding. # 4 plug set at the bottom of the surface casing and last plug set at 200 ft to 0 with good cement returns.

1418989769 2013-11-18 Plug / Plug Back No Violations I am checking this well which has been plugged to surface to see if there are any gas leaks. I found that there are no leaks at this time. The pipe has been cut off and the area needs graded and seeded. Due to the water covering the site it is no possible at this time and most likely will be late spring before it can be passed for final restoration.
1711034272 2013-11-01 Plug / Plug Back No Violations I am on site today to plug this well which has a history of H2S gas problems. Nicks Plugging Service is moving in today and while moving in found that the location does not have a firm bottom and will need a mud boat for the rig to set on. W/O dozer to arrive then spot the rig on the boat an move to the well to begin plugging. 400 PPM of H2S was present at the well head during the rig move. OBriens Safety Service is on site to monitor the H2S and set up a rescue should it be needed.
1894609621 2006-11-24 Production Wells No Violations Check this well as part of a complaint on my home phone of sulphur gas odor in this area .Check well because Brett was on vacation and I had this problem with this well when I had this County .I checked well and found that Sulfide blocker had just been added to well . It still stinks of H2S but there are no leaks .
