0319745601 | 2016-03-08 | Production Wells | Violations Noted | I arrived on location accompanied by inspector Mark Moreland, were I witnessed the well is idle. The well head consisted of 8.625 inch diameter surface casing and 4.5 inch diameter production casing. At the top of the production casing there is a 4.5 inch to diameter to a 2 inch diameter changeover with valve. The valve was in the shut position and the well was not connected to the flow line indicating that the well is not in production. All the production equipment has been removed off location. Previous reports have indicated that plugging permit has been filed for the well, however the permit has expired and the well has not been plugged. Mark contacted 740-452-9326 and left a voice mail in regards to the status of the well. No return call has been made. The well owner/operator shall submit a written plugging schedule to Joe Zollars at 2207 Reiser Avenue SE New Philadelphia, Ohio 44663-3333 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |

I found that Hopewell had removed the 210bbl tank and it appears that they had excavated the contaminated area around the tank. There is still various production equipment and debris in the area. According to Jerry Henderson with Hopewell, they plan to plug the well this summer. They have a plugging permit that will expire on 11-08-15. Updated photographs were collected. |
1365131429 | 2015-03-11 | Production Wells | Violations Noted | On the date of inspection I found the prior entry to the production equipment to be overgrown with brush. There is a 210bbl tank within a partial earthen dike that has been plumbed with a 2 inch line that leads to the roadway and ditch. The 2 inch line is rusted out near the tank valve. Any fluid that was left in the tank would have been free to exit. There is a small amount (approximately 3 feet by 4 feet area) of what appears to be oil contamination. There are several pieces of production equipment debris around the dike. The well is equipped with a gas plunger lift system, 4.5 inch casing, 8.625 inch casing, and rusted out conductor at the surface. All valves are in closed position and flow line is disconnected. Production was last reported in 1999, 2000, and 2001. Before 1999 production was not reported since 1992. There was a faint odor of H2S near the well head at the time of my inspection. The well has an active plugging permit that will expire on 11-08-2015. |

I spoke with the owner of the company, Jerry Henderson. I made him aware of the condition of the well and that the tank appeared to be leaking oil from the bottom. Mr. Henderson informed me that he would have a well tender visit the well as soon as possible to assess the tank, remove it, and clean up the contamination. Mr. Henderson plans to move in to plug the well as soon as the weather will allow him to move equipment on location. I informed him that I would issue a Compliance Notice if the permit to plug and abandon were to lapse. |
