The well does have Hydrogen Sulfide Poison gas sign along with a wind sock at the well head. There is a 210 bbl. Production tank and a horizontal separator being used to produce the well which has a pump jack rods and 1 1/2 diameter tubing. |

0416889695 | 2016-10-03 | Plug / Plug Back | No Violations | I am on site today to witness the plugging of the well bore. On the day of my inspection I witnessed Nicks Well Plugging Services rigged up over the well bore. Specialized Safety Services is also on site with HS2 monitors station around the working perimeter and will be monitoring for H2S gas for the duration of the job. Prior to working on the well bore a safety meeting was held by Randy Tinker (A Specialized Safety Services employee) who informed everyone on location where the muster points where located and what to do in an emergency situation. Air packs where also stationed around the location, incase an emergency rescue needed to be preform. After the safety meeting I witnessed the well was tested for H2S readings. Readings on the backside on the production casing where calculated at 500 plus ppm H2S and 300 plus ppm inside the production casings. NWP then mixed one barrel of Hydrogen Sulfide Blocker to 30 barrels of fresh water and displaced it down the annular spaces. After the well was declared safe to work on, I then witnessed NWP pulling all rods out of the well bore and setting a 30 sack plug through 2.375 inch diameter tubing from 5450-5050 feet. NWP then displaced with 19.5 barrels of fresh water and pulled the tubing out of the well. |

As stated in my previous inspection Nicks Well Plugging Service will be preforming the service work and Specialized Safety Services will be preforming monitoring and providing safety services if needed. Sutton pump is on site this morning and is steaming the production tubing. Prior to working on the well I witnessed Specialized Safety Services packing up with an air pack and gathering H2S readings on the well bore. After evaluating the readings it was discovered there was 2,000 plus parts per million on the backside of the production casing and approximately 1800 ppm inside the production casing. After the readings NWP again treated the well with Hydrogen Sulfide blocker. While treating the well a meeting was held on the bases of plugging the well with limited exposer due to such high readings. Chuck Woods (an Atlas Noble Supervisor) and Mike Moreland (ODNR District Supervisor) agreed that a plug would be set in the lime using perforations and squeezing the cement. This was in an attempt to shut off the H2S producing zone. After theH2S was depleted NWP then ran the 2.375 inch diameter production tubing to a depth of 3900. Appalachian Well Surveys then ran wire line to a depth of 3949 and shot ten perforations in the 4.5 inch diameter production casing. After the perforations where set NWP then ran tubing to 3949 and set a 155 sack from a depth of 3949-3549. Once the plug was displaced NWP then squeezed with 3 barrels of fresh water and shut the well in for over the night. |

As stated in my previous inspections Nicks Well Plugging Service will be preforming the service work and Specialized Safety Services will be preforming monitoring and providing safety services if needed. Prior to working on the well I witnessed Specialized Safety Services packing up with the air packs and taking H2S readings. After evaluating the readings from the well, it was discovered there was no H2S. After the reading NWP then pulled out the tubbing and tagged the plug that was set the previous day with the 4.5 inch diameter ripping tools. The plug was tagged at 3250. Once the plug was tagged we calculated that the H2S producing zone had been successfully plugged off and Specialized Safety Services then left the location. NWP then ripped the 4.5 inch diameter production casing at a depth of 1360 feet. After the casing was ripped NWP then pulled out the production casing to a depth of 1200 feet and set a plug from 1200-980 feet, utilizing 67 sacks of Class A cement and displacing with 15.6 barrels of fresh water. After setting the plug NWP then pulled the casing out to a depth of 200 feet and circulated cement to surface |
1436817034 | 2016-09-02 | Production Wells | Violations Noted | On the day of my inspection I witnessed the well is utilizing a pump-jack for means of production. The well bore consist of 8.625 inch diameter surface casing, 4.5 inch diameter production casing, 2.375 inch diameter production tubing and pump assembly components. A prime mover is in place on the unit. A leak and an odor could was noticed upon my arrival. After getting out of the truck and walking within approximately 20 feet of the well bore my H2S meter detected approximately 42 parts per million of Hydrogen Sulfide Gas. The leak of gas appeared to be coming from the 8.625 inch diameter surface casing. The well is located in a manor were it is proposing a threat to human health and safety. Production equipment for the well bore consists of a 210 barrel production tank, a 30 barrel brine tank and a horizontal separator with a 2 inch diameter meter run. The meter chart indicated the well was in producing order. All production equipment is placed in a dike that measures approximately 40*50 feet at an average height of 3 foot around the perimeter. After my inspection I called Chuck Woods and informed him of my findings. I then called my supervisor Mike Moreland who mandated that the well be plugged in 90 days. |
