0307360196 2016-04-05 Production Wells No Violations I arrived on location to conduct a follow up inspection from a spill that had occurred due to a stuffing box failure. On the day of my inspection I witnessed the contaminated area has been fully excessed and now there are no signs or pollution or contamination; vegetation has yet to be reestablished. The well is utilizing a pump jack for means of production. The well head construction consists of 8.65 inch diameter surface casing, 4.5 inch diameter production casing, 2.75 inch dimeter production tubing and polish rod/rods. Production equipment consists of a 100 bbl. production tank, horizontal separator and a 2 inch diameter meter run. All production equipment is placed inside a dike that measures approximately 30 by 40 feet at an average height of 3 feet. RBDMS records indicate the well is in producing order.
1447881050 2015-11-18 Production Wells No Violations I received a call from Inspector Mark Moreland, who was called and informed that an oil/brine spill had occurred around the pump jack due to a suspected stuffing box failure. Prior to my arrival I called the Ohio Emergency Response line, to notify them of what I had just been informed. When I arrived on location Chris Belch ( Atlas Production Foreman) was on scene. He confirmed to me that the spill had occurred due the stuffing box rubber failure. Chris estimated that approximately 20 gallons of oil/brine had been released due this failure. Prior to my arrival Chris had placed peat sorb and oil absorbent pads down to contain the spill. The effected area from this spill measured 10 foot wide by 10 foot long, and did not leach out of its contained area or into any water source. During my inspection Atlas Energy brought in a steam pressure washer, and a mini excavator. The steam cleaner was used to clean off the contaminated well head and pump jack. Atlas then used the mini excavator to remove the contaminated soil and gravel. The containments was hauled off by Sutton Pump who took the manifest to a Kimble facility.
1462396067 2016-07-14 Production Wells No Violations This well is using a 110 bbl. Tank to produce into and a buried cement vault for brine storage. There is a horizontal separator hooked up to a well operating with a pump jack rods and tubing.
1697627945 2017-10-17 Production Wells No Violations On the day of my inspection I found this walking beam well equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, and rods. Pumping unit power by natural gas. Valves are open. Production equipment includes a 100 barrel steel tank, horizontal seperator with 0.25 inch diameter sales line orfice, meter box with fresh chart, and a concrete brine vault, and domestic gas meter. Production records are current in RBDMS.
