0486064787 2016-05-04 Final Restoration No Violations I arrived on site today accompanied by Inspector Zollars. This well has been plugged and the production equipment has been removed. It meets the requirements for the final restoration to be approved.
0530213284 2016-05-04 Final Restoration No Violations On the day of my inspection accompanied by inspector Mark Moreland, I found that the final restoration has been complete. The site passed final restoration according to ORC 1509.072(B).
0824372027 2017-05-26 Final Restoration No Violations On the day on my inspection I was unable to find a well at the above coordinates. This permit was a lost hole and the rig was skidded over and a new permit was issued. 20189 was the new permit and it has a final restoration. Inspectors Mark Moreland and Joe Zollars completed a final restoration on this site, May 2016. During my inspection, I was approached by the landowner and he confirmed that the well was plugged and that he was happy with the reclamation.
