Specialized Safety Services has H2S monitoring equipment set up around the perimeter of the well. Before starting the service we attended a safety meeting exampling what the work conditions were and where to go in case of an emergency. We also went over the plugging operation and were the plugs will be set. After the meeting I spoke to the company man from Nicks Well Plugging Service, who informed me that he was going to attempt to set the bottom plug today. Later in the day the well conditions prevented Nicks from being able to set the bottom plug. I will be back on site tomorrow to witness the setting of the bottom plug. |

On the day of my inspection, Appalachian Well Services was on site to set perforations inside the 4.5 inch diameter production casing. Appalachian set 10 shots at a depth of 4550 feet, Nicks Well Plugging Service then ran the 2.375 inch diameter tubing to where the perforations where located to set a 200 foot plug from 4550 to 4350 feet to cover up the H2S producing zone. After the plug was set Nicks pulled tubing up to a depth of 3885 and Appalachian shot 10 perforations at 3935. After the perforations, Nicks then ran the tubing back to 3935 and set a 200 foot plug from 3935 to 3735. After the plug was set Nicks pulled the tubing out and ripped the 4.5 inch diameter production casing at a depth of 1243 feet. Production casing was then pulled out of the hole and tubing was ran back to set the next 200 foot plug from 1195-995. After the plug was set tubing was then pulled to 200 feet and cement was then circulated to surface. During the duration of the Job Specialized Safety Services was monitoring the site for H2S and no signs or indications were discovered. |

The plug job will continue tomorrow, when Appalachian Well Services arrives on site to set perforations. 1593327318 | 2016-05-18 | Production Wells | No Violations | I am on site today accompanied by inspector Mark Moreland, were I witnessed the well is utilizing a pump jack for means of production consisting of 8.625 inch diameter surface casing, 4.5 inch diameter production casing, and 2 3/8 inch diameter production tubing. The production tubing valve and production casing valves where in the closed position at the time on my inspection. Production equipment consisted of a 100 bbl. production tank, a 50 bbl. steel brine tank and a horizontal separator with a two inch diameter meter run. All production equipment was placed inside an earthen dike that measured approximately 30*40 feet at an average height of 3 foot. By the well head and the tank battery were wind socks and signs indicating warnings of poisonous gas, however I witnessed no signs of gases at the time of my inspection. |
