0034660038 2012-07-06 Production Wells No Violations On site in response to Complaint # 3869. I met with Tracey Mitchell with Columbia Gas on location at 5pm. Columbia Gas had detected natural gas around the foundations of two homes (832 and 850 Wilson St.) and suspect the well might be the source. The well is located in a 6 by 6 white shed with a 2 diameter vent pointing 15 above the ground. The well is being used for domestic purposes by landowner Chad Kirk. The well has 80 psi of pressure in the lines on the well side of the regulator. A faint odor of gas is present inside the shed with LEL readings above 15 on my gas meter. The well used to heat multiple houses in the area but those lines were disconnected when an alley was being renovated an unknown number of years ago. Mr. Kirks home is the only one currently being supplied with gas from the well. Samples of the gas from around the well were collected by Columbia Gas to be compared with the samples from around the foundations. Well was added to RBDMS in 2009 and Domestic Ownership Papers need to be filed with the Division.

0589581107 2019-04-11 Production Wells No Violations Received text message from owner of this well about desire to register well and to have well plugged at a later date. Inspected well with Stephen Ochs and determined that flow line going to residence at this address has been disconnected. Slight odor of natural gas was observed coming from well. This well currently has a compliance notice to plug well from an inspection Dated 9/10/2014. Inspection # 1410441247 with a Compliance Date of 9/11/2014 was completed. The Remedy Date for this well was 3/11/2015. Advised owner of well about findings and how to begin the process of registering this well.
1345052267 2012-08-09 Production Wells Violations Noted At the time of my inspection there was gas leaking from the annular space between the 6 surface casing and the 8 conductor casing above the surface of the ground. The 6 surface casing does not appear to be cemented back to surface and there are no records for the well located in RBDMS. The depth of the 4 1/2 liner casing is unknown. The well needs registered to a bond and properly insured as an urban well, and tested to ensure proper well construction.
1356635549 2012-12-06 Production Wells No Violations At the time of my inspection the Corrective Action required from the Compliance Notice issued on 8/9/2012 had not been completed. I contacted Mr. Kirk who indicated he was still looking for a contractor to complete the required work and is still completing his ownership paperwork.
1410441247 2014-09-10 Production Wells Violations Noted This inspection was done as a follow-up to Compliance Notice # 1345052267. At the time of the inspection the well is still inside a 5 x 5 shed and being used for domestic purposes at 827 S. Blanchard St. in Findlay. The LEL level inside the shed was at 35 and CH4 at 1.05 indicating the well has a leak somewhere at the surface. The property is owned by Kirk Properties Inc. (2959 SR 23, Alvada 44802) and the tenant is Chad Kirk. Mr. Kirk and Kirk Properties were made aware of their obligations for bonding and insurance on the well back in 2012, but the well has yet to be brought into compliance.

1533574738 2018-08-06 Production Wells No Violations Follow-up to COMPLAINT # 3869 Dated 7/6/2012. Received a call from Matt Pickett, Fire Investigator with the Findlay fire Dept, about a house at 832 Maple St which had stray gas readings. He reported that Columbia Gas of Ohio had been called to the residence at 832 Maple St and reported that stray gas was found around the foundation and that they had turned the gas off. Columbia Gas took readings of the stray gas and determined that the gas was not from third pipeline. The location of this well was found to be in a five foot by five foot building . The building was secured and a slight odor of gas was noticed coming from cracks in the building. Contacted the owner, Chad Kirk, of 827 S Blanchard and he admitted that there was a gas well in the building and that he was using gas from that well at this location. He would not allow access to the building. He did explain that he was the owner of this well and had the mineral rights to this property. The well is not currently registered with the Oil and Gas Division and I explained that it was required to be registered. He did respond that he would look into resolving the gas smell and also register the well with the State of Ohio.

1915125785 2012-08-24 Production Wells No Violations At the time of my inspection I was on location to meet with Mr. Kirk to discuss options for servicing the well to stop the escape of gas from between the 8 drive casing and the 6 surface casing. Mr. Kirk was unable to meet because of a work conflict and he indicated he would call later in the day to set up another time to meet.
