0054855623 2019-05-23 Preliminary Restoration No Violations The purpose of my inspection was a status check on preliminary a restoration. The pad was constructed with disturbed areas restored in the year 2014. In the year 2019 two additional wells were added to the pad. The cuttings from the two cellars and two conductor boreholes were stockpiled on the South side of the pad with vegetation established. The pad slopes appeared to be stable. The well was being prepared for flowback and the wellhead was being assembled. The production equipment was being hauled on to location and arranged on pad. Identification signs and drilling permits were posted at entry. The location meets the requirements for preliminary restoration as per the Ohio Revised Code 1509.072 (A).
0402619805 2019-03-05 Drill / Deepen / Reopen No Violations The purpose of my inspection was a status check on Patterson 584. During my inspection the rig was drilling the 8.5-inch diameter production borehole at 14853 feet measure depth. The kick off point was at 6880 feet and the landing point was at 8225 feet. I spoke with the Ascent Company Man on location and he stated that they would likely be cementing the 5.5-inch diameter production casing Friday morning. The 5.5-inch diameter production casing was evaluated on the pipe racks.
0487796492 2019-04-29 Completion Testing No Violations The purpose of my inspection was a status check on completion operations. During my inspection a workover rig was rigging up on the 5H well to drill out frac plugs and to install production tubing. Sound walls were being removed from location.
0585091353 2019-02-27 Drill / Deepen / Reopen No Violations During my inspection Patterson 584 was on location removing drill pipe from hole. The rig had reached a total depth of 6800 feet within the 12.25-inch diameter intermediate borehole. The 9.625-inch diameter intermediate casing was placed on the pipe racks and a Schlumberger bulk cement truck was on location filling the cement silo.
0590506793 2019-02-21 Drill / Deepen / Reopen No Violations During my inspection Patterson 584 was rigging up on location. The derrick was up, and Grant Smith Trucking continued to move loads in. I spoke with Ascent Company man on location, Michael, he stated that they would like to be drilling the 17.5-inch diameter surface borehole by early tomorrow morning.
0915254003 2019-02-15 Casing String Witnessed No Violations Precision Rathole drilled a 36-inch diameter borehole to 100 feet in depth and installed a 30-inch diameter conductor casing with welded joints. Water was encountered during the drilling process. The conductor weighed 118.65 pounds per foot. During my inspection a pre-job safety meeting was held with all personnel on location and Petroset cementing rigged up on location and pumped 315 sacks of class A cement with 2 percent calcium chloride and displaced with 70.5 barrels of fresh water. Cement returns were established and were used to cement the cellar floor to create a liquid tight cellar.
