0080097290 2019-03-12 Production Wells No Violations I was accompanied by ODNR Southeast Regional Manager Brent Bear and the purpose of our inspection was a status check. During our inspection the well was equipped with a plunger lift system with valves in the open position. The 13.375-inch diameter surface casing had 0 pounds per square inch and the 9.625-inch diameter intermediate casing had 0 pounds per square inch. The production equipment consisted of four sand traps, four gas production units, four horizontal heater treaters, three combustors, one flare, and a compressor. Identification was location at the tank battery and on the wellhead.
0112777900 2019-01-31 Preliminary Restoration No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. A gauge was present on the 9.625-inch diameter intermediate casing reading 0 pounds per square inch and 0 pounds per square inch on the 13.375-inch diameter surface casing. The production equipment consisted of four sand traps, four gas production units, four horizontal heater treaters, three combustors, one flare, and twelve 500-barrel tanks. Produce oil and natural gas from the Jupiter Pad and the Law pad are gathered at this pad (Outlaw). Identification was location at the tank battery and on the wellhead. Two Marathon Petroleum Crude Oil Fleet trucks were loading oil from the tank battery. Location meets the preliminary restoration requirements.
0413215082 2017-10-27 Casing String Witnessed No Violations On the day inspection accompanied by ODNR Inspector Tom Thomas, I witnessed that Precision Rathole had drilled a 30 inch diameter hole to the depth of 118 feet, hitting water at 25 feet and ran 118 feet of 24 inch diameter 94.62 pounds per foot conductor pipe. O-Tex Pumping LLC was onsite and pumped 2 barrels of fresh water, performed a 500 pounds per square inch pressure test and cemented pipe with 290 sacks Class A cement with 2% CaCl and .25 pound per sack of Cello-flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 108 feet with 56 barrels of fresh water with good cement returns of 18 barrels to surface. Mix water was tested revealing chlorides of <32 parts per million, pH 7 and 60 degrees.

0907361628 2017-11-22 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P Flex Rig #531 was drilling ahead on 11.8 pound per gallon mud and 29000 pounds of torque in the lateral with an 8.5 inch diameter bit at 24298 feet with a planned total depth of 28113 feet. The last recorded survey had an azimuth of 329.64 degrees and inclination of 90.93 degrees. I met with Eclipse Consultant Jack Turner and completed a walk around revealing 5.5 inch diameter 23 pound per foot casing was onsite, proper containment was in place and the cement water tanks had been cleaned.

Wyoming Casing Crew ran 226.9 feet of 13.375 inch 61 pound per foot J-55 casing and 1251.49 feet of 13.375 inch 54.5#/ft. J--tested revealing chlorides of <32 parts per million, pH 7 and 54 degrees.

Inspector Doug Bennet was also on site to witness this stimulation stage.
