0802503598 2018-09-10 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. A gauge was present on the 9.625-inch diameter intermediate casing reading 0 pounds per square inch and 0 pounds per square inch on the 13.375-inch diameter surface casing. The production equipment consisted of four sand traps, four gas production units, four horizontal heater treaters, three combustors, one flare, and nine twelve 500-barrel tanks. Identification was location at the tank battery and on the wellhead.
0902384609 2017-12-07 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that H&P Flex Rig #531 was drilling ahead on 11.8 pound per gallon mud in the lateral with an 8.5 inch diameter bit at 10104 feet with a planned total depth of 27892 feet. I met with Eclipse Consultant Neil Ingram and completed a walk around revealing 5.5 inch diameter 23 pound per foot casing was being offloaded.
1114433064 2017-10-25 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Precision Rathole was onsite and setting the cellar. Upon completion Precision will set the cellars on the 34059245170000 Law B 2H and 34059245180000 Outlaw C 7H then commence drilling the conductor boreholes.
1509110189 2017-10-27 Casing String Witnessed No Violations On the day inspection accompanied by ODNR Inspector Tom Thomas, I witnessed that Precision Rathole had drilled a 30 inch diameter hole to the depth of 120 feet, hitting water at 45 feet and ran 120 feet of 24 inch diameter 94.62 pounds per foot conductor pipe. O-Tex Pumping LLC was onsite and pumped 2 barrels of fresh water, performed a 500 pounds per square inch pressure test and cemented pipe with 290 sacks Class A cement with 2% CaCl and .25 pound per sack of Cell0-flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 110 feet with 57 barrels of fresh water with good cement returns of 17 barrels to surface. Mix water was tested revealing chlorides of <32 parts per million, pH 7 and 60 degrees.

During my inspection the well was equipped with a plunger lift system with valves in the open position. A gauge was present on the 9.625-inch diameter intermediate casing reading 0 pounds per square inch and 0 pounds per square inch on the 13.375-inch diameter surface casing. The production equipment consisted of four sand traps, four gas production units, four horizontal heater treaters, three combustors, one flare, and twelve 500-barrel tanks. Produce oil and natural gas from the Jupiter Pad and the Law pad are gathered at this pad (Outlaw). Identification was location at the tank battery and on the wellhead. Two Marathon Petroleum Crude Oil Fleet trucks were loading oil from the tank battery. Location meets the preliminary restoration requirements.
