During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Annular pressure monitoring was present with each valve identified with casing information. The production equipment consisted of eight 400-barrel tanks, two vapor recovery towers, five sand traps, five gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of eight 400 barrel tanks, two vapor recovery towers, five sand traps, five gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.

During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The 13.375-inch diameter casing had 267 pounds per square inch and the 9.625-inch diameter casing had 167 pounds per square inch. The production equipment consisted of eight 400-barrel tanks, two vapor recovery towers, five sand traps, five gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
1505717157 2017-09-18 Casing String Witnessed No Violations

-55 casing 2.7 feet above KB. Falcon Drilling Rig #23 washed down the last 5 joints of casing pumping a total of 280 barrels of fresh water. O-Tex Pumping LLC was onsite and without their cement plug head. Cement head arrived at 07:20 without the cement head crossover bowl. ODNR Inspector Tom Thomas joined me on location for the job. Upon arrival of the swedge at 10:44 O-Tex pumped 5 barrels of fresh water and performed a 2000 pound per square inch pressure test. O-Tex pumped 35 barrels of gel and cemented casing with 1385 sacks of Class H cement with 1.5% Calcium Chloride, .2% X--flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement weight was verified by pressurized mud scales and was displaced with 192 barrels of fresh water. Had good cement returns of 70 barrels, plug was landed at 678 pounds per square inch, pressured to 1242 pounds per square inch for 5 minutes and the floats held with 1 barrel back Mix water was tested revealing chlorides of <32 parts per million, pH 7 and 64 degrees.

1505770966 2017-09-18 Casing String Witnessed No Violations On the day of my inspection, I accompanied ODNR Inspector Doug Bennett, and I witnessed that Falcon Drilling Rig #26 had drilled a 17.5 inch diameter hole to the depth of 1315 feet. Wyoming casing crew ran 1308.7 feet of 13.375 inch diameter 61 pounds per foot J-55 casing 2.7 feet above KB. Falcon Drilling Rig #23 washed down the last 5 joints of casing pumping a total of 280 barrels of fresh water. O-Tex pumped 5 barrels of fresh water and performed a 2000 pound per square inch pressure test. 1512136943 2017-12-01 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #577 was drilling ahead in their 8.5 inch lateral on 12.3 pound per gallon mud in the lateral with an 8.5 inch diameter bit at 16362 feet with a planned total depth of 17425 feet. I was notified on 11/28/17 that they had a 2-3 foot flare at 11:03 PM at 8190 feet. I met with Ascent Consultant Burl Miller and completed a walk around revealing a 1 foot flare with 4596 units of gas measured.
