0103197564 2018-07-06 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of eight 400 barrel tanks, two vapor recovery towers, five sand traps, five gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
0140652958 2018-04-12 Completion Testing No Violations The purpose of my inspection was to follow up on complaint number 5817. During my inspection Halliburton and Allied Horizontal Wireline Services were on location. Halliburton was completing hydraulic stimulation operations on stage 54 out of 91 on the Gil 1H 3405924507 and stage 45 out of 61 on the Gil 3H 3405924508. I discussed operations with Ascent Company man Matt Bollinger. Mr. Bollinger stated that operations have been moving forward without issue.

I witnessed the 30 inch diameter conductor pipe had been set at 100 feet and cemented on Monday. Precision Rathole was onsite drilling the 20 inch diameter mouse hole with pipe to run upon completion.

I met with Ascent Consultant Cecil Lovell reviewed the 2-stage intermediate casing cement job and then completed a walk around.

1511288089 2017-11-21 Casing String Witnessed No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #577 had reached total depth in their 8.5 inch lateral at 17708 feet and 5.5 inch diameter 23 pound per foot P-110 casing was set at 17718.77 feet 10.8 feet above KB. KB= 26 feet. O-Tex Pumping was onsite to cement casing and the proposed plan includes: pump 5 barrels of fresh water, pressure test at 8000 pounds per square inch, pump 75 barrels of 13.5 pound per gallon barite spacer, pump 10 barrels of fresh water, 2845 sacks (640barrels) of 15.2 pound per gallon Class H cement with additives, displacing cement with 374.4 barrels, landing the plug and pressuring up 500 pounds per square inch over for 5 minutes.
1627654346 2017-09-14 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Drilling Rig #26 had drilled a 17.5 inch diameter hole to the depth of 1317 feet and Wyoming casing crew ran 1310.48 feet of 13.375 inch 61 pounds per foot J-55 casing 4.5 feet above KB. Falcon Drilling Rig #23 washed down the last 9 joints of casing pumping a total of 220 barrels of fresh water. Mix water was tested revealing chlorides of 32 parts per million, pH 7 and 62 degrees. O-Tex Pumping LLC was onsite pumped 5 barrels of fresh water and performed a 2500 pound per square inch pressure test. O-Tex pumped 35 barrels of gel and cemented casing with 1385 sacks of Class H cement with 1.5% Calcium Chloride, .2% X--flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement weight was verified by pressurized mud scales. O-Tex shut down and released the plug. 25 barrels into displacement fluid locked up and kicked out the pumps at 2500 pounds per square inch. Multiple unsuccessful attempts were made to re-establish displacement. O--Tex was unsuccessful at re-establishing displacement and shut down. A meeting was held discussing calculations, estimated top of cement and a plan forward with Ascent Consultants Michael Rawlings, Mark Smith and Ascent Cement Guru Mark Henderson. Plan forward included: run 1 inch diameter tubing tagging cement in the annulus, gain circulation with water, top out cement to surface, wait on cement to reach 500 pounds per square inch compressive strength, drill out cement inside casing with 12.25 inch diameter bit to approximately 10 feet from the float shoe and load hole with water and run cement bond log across casing with KLX Energy Services.
