0802152018 | 2018-07-06 | Preliminary Restoration | No Violations | The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of nine 400 barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. |
1058338673 | 2018-02-22 | Completion Testing | No Violations | On the day of my inspection I witnessed Deep Well Service Stand Alone Snubbing Unit # 11 (DWS # 6) was drilling out the last frac plug at 19017 feet with a 4.5 inch diameter tricone bit. Tetra Services was onsite controlling well pressure at 1100 pounds square inch. DWS # 6 will trip out and run 2.875 inch 6.5 pound per foot L-80 tubing and set it at 7880 feet which is at 70 degrees. |
1119052303 | 2018-09-12 | Production Wells | No Violations | The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges were present on all casing strings. The 9.625-inch diameter intermediate casing had 1300 pounds per square inch and the 13.375-inch surface casing had 25 pounds per square inch. The production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and two combustors. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. |
1167288249 | 2018-05-17 | Production Wells | No Violations | The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The wells on the pad are in the process of flowing back. The production equipment consisted of nine barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. |
1193565338 | 2017-11-21 | Completion Testing | No Violations | On the day of my inspection I witnessed Halliburton Services complete stage 42 of 75 with a breakdown pressure of 6022 pounds per square inch, an average treating pressure of 8348 pounds per square inch, an average rate of 78.61 barrels per minute and a total of 247744 pounds of proppant pumped. I completed walk around with Ascent Consultant Matt Bollinger revealing proper secondary containment was in place. |
1298177547 | 2017-05-22 | Casing String Witnessed | No Violations | The purpose of my inspection was to witness 1211 feet of 13.375 inch diameter surface casing cemented within a 17.5 inch diameter borehole that was 1225 feet in depth. During my inspection I witnessed Falcon Drilling Rig 26 run the last two joints of casing with Wyoming Casing Crew, and O-Tex rigging up. A pre-job safety meeting was held with all personnel on location. O-Tex pumped 35 barrels gel spacer, and 284 barrels of Class H cement with 1.5% Calcium Chloride weighing 15.6 pound per gallon and a yield of 1.19 cubic foot per sack. Cement was weighed in pressurized mud scales confirming the weight of 15.6 pound per gallon. The plug was launched and displaced with 178.6 barrels fresh water. Gel circulated to surface 73 barrels into displacement and circulation was maintained bringing 70 barrels of cement to surface. A cement sample pulled from the returns was weighed in pressurized mud scales and the results were 15.6 pound per gallon. The plug was bumped and held for 5 minutes and the floats held. |
1494062859 | 2017-05-06 | Casing String Witnessed | No Violations | On the day inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet, hitting water at 40 feet and ran 100 feet of 30 inch 118.65 pounds per foot conductor pipe. O-Tex Pumping LLC was onsite and performed a 200 pounds per square inch pressure test, gained circulation with 2 barrels of fresh water and cemented pipe with 330 sacks Class H cement with 1.5% Calcium Chloride, .2% X--flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 90 feet with 74.8 barrels of fresh water with good cement returns of 25 barrels to surface. Mix water was tested revealing chlorides of 54 parts per million, pH 7 and 59 degrees. |
1502719291 | 2017-08-14 | Drill / Deepen / Reopen | No Violations | Inspection today was to follow up from a phone call Sunday 8/13/17 from Ascent Consultant Josh Delso where he stated that 10 gallons of diesel was spilled on containment and .5 gallons had breached containment. Upon arrival I met with Mr. Delso and reviewed the incident an Air Drilling Associates hand was having trouble with fuel delivery on the air package and left a pressure relief valve cracked resulting in the spill. We then visited the area of impact revealing that all contamination had removed, rig mats had been pulled up with repairs to the underlying containment being made with the rig mats re-installed and no impact offsite was revealed. Ascent representatives will be monitoring the location and pulling water from the sumps for any residual fuel that has not been recovered. |
1510069221 | 2017-11-07 | Completion Testing | No Violations | On the day of my inspection I witnessed Halliburton Services complete stage 12 of 75 with a breakdown pressure of 10035 pounds per square inch, an average treating pressure of 10035 pounds per square inch, an average rate of 64.3 barrels per minute and a total of 247478 pounds of proppant pumped. I completed walk around with Ascent Consultant Malcom Trahan revealing proper secondary containment was in place. |
