0519231818 2017-09-08 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #584 had just landed their curve 8210 feet and was drilling ahead in the lateral with an 8.75 inch diameter on 12.5 pound per gallon mud. Patterson UTI Drilling Rig #584 will drill to approximately to 8410 feet, trip out and pickup an 8.5 inch Schlumberger Orbit tool and trip back in. Patterson UTI Drilling Rig #584 will continue drilling to a planned total depth of 19435 feet. I completed walk around with Ascent Consultant Alan Rice.
1048662132 2019-02-07 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges were present on all casing strings. The 13.375-inch diameter surface casing had 70 pounds per square inch and the 9.625-inch diameter intermediate had 355 pounds per square inch. The production equipment consisted of nine 400-barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry. Ascent well tender was on location along with two Wise tanker trucks that were loading produce fluids from the tank battery.

1146234151 2018-07-06 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. The production equipment consisted of nine 400 barrel tanks, two vapor recovery towers, six sand traps, six gas production units, three horizontal heater treaters, one compressor, one vertical vent scrubber, and one combustor. The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
1460819265 2017-08-31 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #584 had drilled out the 13.375 inch diameter float equipment, drilled 10 feet of new wellbore to 1236 feet and successfully completed a Formation Integrity Test (FIT). I met with Ascent Drilling Superintendent Richard Thompson and Ascent Consultant Alan Rice and inspected the southeast sump revealing no sheen at this time. Ascent will continue to pump out the sump until results are satisfactory.
1493750066 2017-05-02 Casing String Witnessed No Violations On the day inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet, hitting water at 40 feet and ran 100 feet of 30 inch 118.65 pounds per foot conductor pipe. O-Tex Pumping LLC was onsite and pumped 2 barrels of fresh water, performed a 150 pounds per square inch pressure test, gained circulation with 2 barrels of fresh water and cemented pipe with 330 sacks Class H cement with 1.5% Calcium Chloride, .2% X--flake weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 90 feet with 74.8 barrels of fresh water with good cement returns of 25 barrels to surface. Mix water was tested revealing chlorides of 54 parts per million, pH 7 and 59 degrees.
1494720175 2017-05-13 Casing String Witnessed No Violations On the day of my inspection I witnessed that Falcon Drilling Rig #26 had drilled a 17.5 inch diameter hole to the depth of 1225 feet and Wyoming casing crew ran 1218 feet of 13.375 inch 61 pounds per foot J-55 casing 4.7 feet above KB. Falcon Drilling Rig #23 washed down the last 4 joints of casing. O-Tex Pumping LLC was on site and gained circulation with 5 barrels of fresh water, 35 barrels of gel, 10 barrels of fresh water and cemented casing with 1350 sacks of Class H cement with 1.5% Calcium Chloride, .2% X--flake weighing 15.6 pounds per gallon and a yield of 1.19 cubic foot per sack. Cement weight was verified by pressurized mud scales at 15.6 pounds per gallon with the truck densometer off; cement was run at 16.7 pounds per gallon on the truck. Cement was displaced with 168 barrels of fresh water. Had good cement returns of 68 barrels, plug was landed at 540 pounds per square inch, pressured to 1100 pounds per square inch for 5 minutes and the floats held with1 barrel back Mix water was tested revealing chlorides of 39 parts per million, pH 7 and 55 degrees.

The identification was posted at the well, tank battery, and entry. Laminated copies of the Drilling permits were posted at entry.
1730280015 2017-10-16 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Allied Horizontal Wireline Services (AHWS) had just completed a gauge run on the 5.5 inch diameter casing. The tubing wellhead was being installed and upon completion AHWS will run a bond log on the casing. Location preparation is continuing for upcoming frac.
