0099435627 2017-05-31 Casing String Witnessed No Violations On the day of my inspection I witnessed that H&P Flex Rig #531 had drilled a 17.5 inch diameter hole to the depth of 1360 feet. Wyoming Casing Crew ran 1310 feet of 13.375 inch diameter 54.5 pound per foot J--in cement head for 1 hour and re-opened with no fluid returning. Mix water was tested revealing chlorides of less than 250 parts per million, pH 7 and 61 degrees.

I met with Eclipse Consultants Ryan Bartholomew and Matt Amos who stated that rig mats and containment will be laid today. Precision Rathole will be onsite tomorrow with in preparation of spudding of the well this week.
0217399665 2018-07-19 Preliminary Restoration No Violations The purpose of my inspection was a status check on a preliminary restoration. During my inspection Deep Well Services Rig 4 was rigged up on the Yost B5H. I spoke with Eclipse Company man and he stated that they were in the process of installing a plunger lift system. Two 500-barrel frac tanks and one 500-barrel gas buster were being utilized. All casing strings were equipped with a pressure gauge and were being monitored. The area effected by the construction of the pad has been restored with vegetation established. The location meets preliminary restoration requirements listed under ORC 1509.072(A).

The production equipment included a GP unit, a line heater and a sand trap. Fluids are separated at the GP unit and then transferred to the Hayes CGF.
0911935550 2017-03-18 Casing String Witnessed No Violations On the day inspection I witnessed that Precision Rathole had drilled a 30 inch diameter hole to the depth of 120 feet, hitting water at 85 feet and ran 120 feet of 24 inch diameter 94.62 pounds per foot conductor pipe. Stingray Pressure Pumping was onsite and pumped 2 barrels of fresh water, performed a 500 pounds per square inch pressure test, gained circulation with 20 barrels of fresh water, pumped 15 barrels of gel, pumped 10 barrels of fresh water and cemented pipe with 290 sacks Class A cement with 2% CaCl and .25 pound per sack of Celluseal weighing 15.6 pounds per gallon and a yield of 1.2 cubic foot per sack. Cement was weight was verified with pressurized mud scales and displaced to 110 feet with 57.75 barrels of fresh water with good cement returns of 13 barrels to surface. Mix water was tested revealing chlorides of 54 parts per million, pH 7 and 53 degrees.

During my inspection the well was equipped with a plunger lift system with valves in the open position on the production tubing. Two production flow lines are attached to the well, one to produce from the tubing and one to produce from the production casing. The 13.375-inch diameter surface casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 0 pounds per square inch, the 5.5-inch diameter production casing had 890 pounds per square inch, and the 2.875-inch diameter tubing had 400 pounds per square inch. The production equipment consisted of eight sand traps, eight gas production units, and three 400-barrel tanks. The produced water was being separated on pad and distributed in the 400-barrel tanks. The oil and gas were being gathered at the Eclipse Hayes CGF pad via pipeline. Identification was located at the wellhead.
2138035983 2017-06-27 Completion Testing No Violations On the day of my inspection I met with Eclipse Consultant Dustin Moore and reviewed upcoming frac schedule which will consist Halliburton fracing 63 stages beginning upon the completion of the 34059244950000 Outlaw C 11H and the 34059244940000 Great Scott 3H. The well will be zipper fraced with the 34059244960000 Yost C 1H. I completed walk around confirming at this time containment is being completed for the installation of the onsite produced water tanks. Upon completion tanks will be set and offloading of sand will begin with commencement of frac to follow on the Outlaw and the Great Scott.
