0051900327 2018-02-01 Casing String Witnessed No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #584 had drilled a 17.5 inch diameter hole to the depth of 1290 feet. Wyoming Casing Crew ran 1293.94 feet of 13.375 inch 61 pound per foot J-55 casing set at 3.9 feet above KB. KB height is 25 feet above ground level. Schlumberger was on site and loaded their lines with 5 barrels of fresh water, performed a low pressure test at 500 pounds per square inch and performed a high pressure test at 3000 pounds per square inch. Schlumberger gained circulation with 25 barrels of gel and cemented, 10 barrels of fresh water casing with 1358 sacks of Class H cement 15.6 pounds per gallon, 1.23 cubic foot per sack cement with 3% D044 Salt, 4% S001 CaCl, .125 pound per sack Flake and .020 gallon per sack D047 Antifoam. Cement weight was verified by pressurized mud scales and displaced with 190 barrels of fresh water. Cement was circulated to surface with good returns of 65 barrels, plug was landed at 500 pounds per square inch and pressured to 1015 pounds per square inch for 5 minutes and the floats held with 1 barrel back. Mix water was tested revealing chlorides of 39 parts per million, pH 6 and 62 degrees.
0315067854 2018-04-12 Completion Testing No Violations The purpose of my inspection was to follow up on complaint 5815. The pad was named Betts and consisted of 3 wells. During my inspection I witnessed that Deep Well snubbing unit 11 was rigged down and moved off of location, the production tubing has been ran, and the wells 3405924483 and 3405924484 are being prepared for flow back. The production equipment consisted of a sand trap, three gas production units, one vapor recovery tower, two compressors, one vent scrubber, one 400 barrel gas buster tank, and one combustor. There was not any smoke coming from the combustor during inspection.
0614220307 2018-02-05 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #584 had reached total depth in their 12.25 inch intermediate section at 7091 feet and loaded the wellbore with 10 pound per gallon mud. Patterson UTI Drilling Rig #584 began tripping drill pipe from the wellbore and upon completion will run 9.625 inch diameter 40 pound per foot casing with a 2-stage cement job to follow.

1387366101 2016-10-04 Casing String Witnessed No Violations The purpose of my inspection today is to witness a 100 feet of a 30 inch diameter conductor being cemented into a 36 inch diameter hole. 10 barrels of fresh water was pumped ahead of 71 barrels of Class A cement weighing 15.6 pounds per gallon. Cement returns were established. During the drilling of this 36 inch conductor hole, no water was found.

The area effected by the pad construction has been restored with vegetation coverage. The lease road and well pad consisted of a gravel surface with no erosion issues. The location has met preliminary restoration requirements written within 1509.072 (A). The identification was posted at the well, tank battery, and entry.
