0018993380 2015-05-10 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Precision Rathole had drilled a 36 inch diameter hole to the depth of 100 feet and ran 100 feet of 30 inch 118.65#/ft. conductor pipe. Allied Oil & Gas Service was on site gained circulation, pumped 20bbls of gel and cemented pipe with 310 sacks Class A cement with 2% CaCl and .25#/sk. of poly-flake with a weight of 15.6ppg and a yield of 1.2cuft/sk. Cement weight was verified by pressurized mud scales. Cement was displaced to 90 feet with 75bbls of fresh water and had good cement returns of 15bbls to surface.

1506201379 2018-07-20 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. Gauges were present on all casing strings. The production equipment consisted of three sand traps, three gas production units, one vertical heater treater, one compressor, one vapor recovery tower, one vertical vent scrubber, one combustor, and five 400-barrel tanks.
1519281390 2018-02-22 Casing String Witnessed No Violations On the day of my inspection I witnessed Patterson UTI Drilling Rig #584 had drilled a 17.5 inch diameter hole to the depth of 1142 feet. Wyoming Casing Crew ran 1145.52 feet of 13.375 inch 61 pound per foot J-55 casing set at 3.5 feet above KB. KB height is 25 feet above ground level. Schlumberger was on site and loaded their lines with 5 barrels of fresh water, performed a low pressure test at 1000 pounds per square inch and performed a high pressure test at 3000 pounds per square inch. Schlumberger gained circulation with 25 barrels of gel and cemented, 10 barrels of fresh water casing with 1236 sacks of Class H cement 15.6 pounds per gallon, 1.23 cubic foot per sack cement with 1.4% D044 Salt, 3.8% S001 CaCl, .125 pound per sack Flake and .020 gallon per sack D047 Antifoam. Cement weight was verified by pressurized mud scales and displaced with 167.8 barrels of fresh water. Cement was circulated to surface with good returns of 70 barrels, plug was landed at 610 pounds per square inch and pressured to 1120 pounds per square inch for 5 minutes and the floats held with 1 barrel back. Mix water was tested revealing chlorides of 45 parts per million, pH 6 and 62 degrees.
1528984876 2018-06-14 Completion Testing No Violations On site inspection accompanied by ODNR North Region Emergency Response Coordinator Andrew Adgate. On this day it was observed that the drilling and completion process has been completed, the production tree well head assembles have been installed and crews are on site installing the production equipment. A production fluid facility has been erected with 5 steel construction 400 barrel production tanks present inside a polymer lined steel construction dike area. There are a total of 3 wells on the pad present that have been completed
