We met with Eclipse Consultant Billy Shafer and completed walk around.
0370010488 2018-07-13 Completion Testing No Violations The purpose of my inspection was a status check on drill out operations. During my inspection Deep Well Snubbing unit #10 was rigged up on location drilling out frac plugs at a measured depth of 18773 feet. A walk around inspection was completed with Eclipse Company Man. Annular pressure monitoring was present. Production equipment was being assembled on the East Side of the pad.
0517844844 2017-09-06 Casing String Witnessed No Violations On the day of my inspection I witnessed H&P Flex Rig #608 had reached total depth in the 12.25 inch intermediate section at 6955 feet. Wyoming Casing Crew had run 9.625 inch diameter 40 pound per foot P-110 casing set at 6910.58 feet. Casing was set 3.8 feet above KB which is 26.5 feet. DV tool was set at 1859.9 feet. Schlumberger was onsite to complete a 2--established circulation with 5.3 barrels of fresh water, pumped 24 barrels of MPE, then pumped 255 sacks of Surface Set 15.6 pounds per gallon 1.21 cubic foot per sack cement with .4% B348 FLAC, .55% DO13 Retarder, .02 gallon per sack D047 Antifoam, .12 gallon per sack D801 Retarder, .05 gallon per sack D145A Dispersant and .4% D202 Dispersant. Cement was displaced with 10 barrels of fresh water, 126 barrels of pound per gallon mud and 15 barrels of fresh water. Plug was landed at 250 pounds per square inch and pressured up to 1550 pounds per square inch to ensure DV tool had closed plug held
0685085320 2019-01-31 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a production tree style wellhead with valves in the open position. A gauge was present on the 9.625-inch diameter intermediate casing reading 1100 pounds per square inch and 0 pounds per square inch on the 13.375-inch diameter surface casing. The production equipment consisted of five sand traps, five gas production units, three frac tanks, two 400-barrel tanks, and a triethylene glycol dehydration unit. Identification was located on the tank battery. Produced water was being separated here and stored on pad. The produced oil and produced natural gas were being transported to the Outlaw pad for gathering via pipeline.

Upon arrival I met with Doug and EHS Field Representative Mike Hartshorn, I witnessed that at the end of the existing gabion baskets another slip has developed on the Northeast side of the access road. CTL will be excavating and continuing with the gabion baskets toward the well pad and installing drainage.

The production equipment consisted of five sand traps, five gas production units, three 500-barrel frac tanks, and two 400-barrel tanks. Roustabouts were on location assembling the triethylene glycol dehydration unit on the South East side of the pad beside the pipeline tie in.
