0156899111 2019-05-20 Production Wells No Violations The purpose of my inspection was a status check. On 4/25/2019 inspection found two slips have developed along the access road and on the topsoil stockpile. I contacted ODNR horizontal well pad Engineer Mark Atkinson and made him aware of the problem areas. During my inspection today the slips remain but have not increased in severability. The current slips will not effect pad stability and performance.
0229986434 2016-09-21 Completion Testing No Violations On the day of my inspection I witnessed the Sweco tubing head installation and successful pressure testing at 10,000 pounds per square inch. Nine wireline will complete bond log on the 5.5 inch production casing upon completion of installation of FMH frac valve. I completed walk around with Consultant Cristo Giannareas revealing that secondary containment was in place with plans to begin toe prep tomorrow.
0292375143 2017-01-10 Completion Testing No Violations The well was equipped with a plunger lift system with valves in the closed position with annular pressure monitoring in place.
