The production equipment included 8-400 barrel tanks and 1-50 barrel tank within containment, a vapor recovery tower, a vapor blow case, a 3-phase vertical separator, 2-compressor units, a vertical heater treater, a gas combustor and back-up flare stack all shared with the Miller 1-25H and Miller 2-25H. Also present a horizontal line heater, a vertical separator and a sand trap. The well was equipped with a production tree with valves in the open position with 300 pounds per square inch on the tubing and 550 pounds per square inch on the casing. Annular pressure monitoring was present along with proper identification and production records being current.

0819249999 2014-08-12 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Brandon Trigg, I witnessed that the sump valves were open and draining unattended as the rig was rigging down. Discharge water was tested with Quantabs which read 130ppm. I received phone call while on site from Drilling Superintendent Scott Treadway and discussed the sumps not being monitored. I stated that someone needs present to monitor water while discharge is taking place. At that time the tool pusher was instructed to close valves and lock in the closed position.

The production equipment included 8-400bbl tanks and 1-50bbl tank within containment, a vapor recovery tower, a vapor blow case, a 3-phase vertical separator, 2-compressor units, a vertical heater treater, a gas combustor and back-up flare stack all shared with the Miller 1-25H and Miller 2-25H. Also present a horizontal line heater, a vertical separator and a sand trap. The well was equipped with a production tree with valves in the open position with 550psi on the tubing and 950psi on the casing. Annular pressure monitoring was present along with proper identification and production records being current. Field tests conducted on the sump water revealed a pH of 6 and chlorides at 68ppm.
1326965664 2014-05-29 Casing String Witnessed No Violations On the day of my inspection I witnessed Rocky Mountain Drilling Inc. drilled a 32 inch diameter hole to the depth of 110 feet and ran 110 feet of 26 inch 102.63#/ft. conductor pipe. Stingray Cementing was on site and gained circulation with 56bbls of fresh water. Stingray pumped 15bbls of gel, 10bbls of fresh water and cemented pipe with 240 sacks of class A cement. Good returns of 8bbls and cement stayed surface.

1618369974 2019-05-02 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with a plunger lift system with valves in the open position. The production casing and production tubing had separate production flow lines. Gauges were present on all casing strings. The 13.375-inch diameter production casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 100 pounds per square inch, the 5.5-inch diameter production casing had 400 pounds per square inch, and the production tubing had 350 pounds per square inch. The production equipment included eight 400 barrel tanks, one 50 barrel tank , a vapor recovery tower, a vapor blow case, a 3-phase separator, two compressors, a vertical heater treater, a gas combustor, back-up flare stack, a horizontal line heater, a vertical separator, and a sand trap.
1768070630 2014-07-30 Drill / Deepen / Reopen No Violations On the day of my inspection accompanied by ODNR Inspector Justin Smith, I witnessed that Nomac Drilling Rig #311 was drilling an 8.5 inch diameter lateral hole at a depth of 10170 on 10.5# mud with a planned td of 16190 feet. We met with Company Men Jimmy Stewart and Chuck Restelle no issues at the time of inspection.
2002352495 2015-03-16 Preliminary Restoration No Violations On the day of my inspection I witnessed that the well is online and equipped with a production tree with valves in the open position. Production equipment included a sand trap, a line heater, a vapor recovery system, 2 compressor units 2 separators, a combustor unit, a flare stack and a total of 8-400bbl tanks and 50bbl tank all within containment and shared with the 1-25H and 2-25H. A pig launcher is located on northwest corner of the pad. The site passed preliminary restoration according to ORC 1509.072A.
