During my inspection the well was equipped with 7-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a tubing head, 2.375-inch diameter tubing, and rods. Production valves were in the closed position and there were no lines attached. A gauge was present on the 4.5-inch diameter production casing reading 1400 pounds per square inch. Identification was posted on the wellhead. Production records were current with M&R Investments submitting reports of zero production for the year 2017. I called and left a voicemail with M&R Investments Eddie Moss to ask him if they had any plans for the well in the foreseeable future and to make him aware that the production casing pressure was at 1400 pounds per square inch. Accompanied today by ODNR Inspector Rick Brown.

Formation Cementing will be cementing the conductor back to surface with 15.6 class A cement.
1440436495 2015-08-24 Production Wells No Violations At 8:53pm Saturday 8/22/14 I received phone call from EHS Supervisor Bruce Carpenter stating that Antrim Fired Department was called out in regards to a gas leak and the well tender responded and found the stuffing box was leaking. The well tender made repairs and no fluids were released. On the day of my inspection I took readings with ODNR Honeywell PhD6 meter revealed no gas was present at the wellhead. Production equipment included a 100bbl tank, a 30bbl brine tank and a drip all within an earthen dike. The rotary meter and separator have been removed. The well was equipped with 7 inch casing, 4.5 inch casing, 2.375 inch tubing, rods and a pumping unit with no motor present and all valves in the closed position at the wellhead. Well identification was present and the production records were current with zero oil and gas reported. I called Bruce Carpenter for an update on the status of the well.

Identification was present with the contact information being the last owner of record and reviewing the production records revealed zero oil and gas production reported for 2015, 2014 and 2013. The well owner/operator shall submit a written plugging schedule to Doug Bennett at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

2123699090 2013-04-17 Drill / Deepen / Reopen No Violations On the day of my field inspection I witnessed the crew with G and L Drilling finish running 1018 of new seven inch surface casing in the well bore, then I witnessed one hundred seventy five sacks of class A cement with twelve barrel of cement returns on the seven inch surface casing. The pit was constructed with a flow ditch from the well bore and lined with four mill plastic.

The production equipment included 1-500 barrel tank and 11-400 barrel tanks within containment, a vapor recovery tower, a vapor blow case, a compressor unit, a vertical heater treater, a 50 barrel sand tank within containment, a gas combustor and back-up flare stack all shared with the Karen 23-9-7 2H, Karen 23-9-7 3H and Karen 23-9-7 4H. Also present a horizontal line heater, a vertical separator and a sand trap. The well was cycling and equipped with a plunger lift system with valves in the open position with 250 pounds per square inch on the tubing and 400 pounds per square inch on the casing with annular pressure monitoring in place. Identification was present and production records were current.
