0385566441 2015-06-18 Production Wells No Violations On the day of my inspection I witnessed that the well was equipped with 7 inch casing, 4.5 inch casing reduced to 2 inches with the valve in the closed position. No production equipment was present with an active plugging permit on file dated 4/20/15. Call was made to Eclipse Conventional Production Superintendent requesting an update on upcoming plugging.
1123998512 2015-11-13 Plug / Plug Back No Violations On the day of my inspection I witnessed Wrights Well Service had tripped 2.375 inch tubing to 4309 feet with Formation Cementing Service onsite. Formation unsuccessfully tried to clear tubing and then hooked up on the 4.5 inch casing reverse circulating on the tubing freeing up the blockage. Formation then regained circulation and pumped 62.3bbls of gel ahead of a 90 sack 500 foot Class A bottom plug from 4309 feet to 3809 feet. Cement was displaced with 6.3bbls of gel and 8.3bbls of fresh water. WWS tripped 2.375 inch tubing to 2269 feet and Formation gained circulation and pumped a 26 sack 200 foot isolation plug from 2269 feet to 2069 feet. Cement was displaced with 8bbls of water. Cement calculated top is at 870 feet no bond log was provided. WWS unsuccessfully tried to obtain telegraph at 800 feet and Eclipse representatives requested to try at 700 feet which was granted and casing was not ripped. Eclipse requested to try at 600 feet which was not granted and I stated they needed to make a good faith effort off the calculated top. WWS was not able to gain telegraph and rip casing. 1298459352 2015-01-14 Preliminary Restoration No Violations On the day of my inspection I witnessed that the well was equipped with 7 inch casing, 4.5 inch casing reduced to 2 inches with the valve in the closed position. No production equipment was present and the site passed preliminary restoration according to ORC 1509.072A.
1447333458 2015-11-12 Plug / Plug Back No Violations On the day of my inspection I witness Wrights Well Service tag the cement packer at 2180 feet with 2.375 inch tubing and a 3.875 inch tricone bit. Formation Cementing Services was onsite with their pump truck to provide fluid for WWS power swivel. Formation gained circulation on casing and WWS began drilling out the packer. Upon completion of drilling out of the packer the well began to circulate gas and oil. WWS shut well in and installed additional liner around the wellhead along with a steel flowline leading to the pit. Formation regained circulation killing the well. WWS will trip tubing to bottom and commence plugging of well tomorrow.

1535555644 2016-06-02 Plug / Plug Back No Violations On the day of my inspection I met with Eclipse Resources EHS Superintendent Bruce Carpenter at the site and reviewed the restoration plans which will commence upon the completion of 34059242590000 Hazel Crawford 1. The land owner is waiving the access road and Eclipse will be providing a land owner waiver.
1631621885 2016-05-23 Final Restoration No Violations On the day of my inspection I witnessed the restoration of the site was not completed with the location roughed in but no erosion control present, erosion was developing on location and the access road not restored. I placed a call to Eclipse EHS Field Representatives on 3/21/16 advising them that I needed an update on when the restoration would be completed. I received return calls on 3/22/16 and 4/11/16 from representatives stating the restoration would be completed prior to the due date of 5/16/16. Eclipse EHS Supervisor Bruce Carpenter called and stated that he was working on getting the restoration completed along with a land owner waiver for the access road. Mr. Carpenter stated that he would have a detailed plan for me be the end of the week. I stated that the restoration needs to meet ORC 1509.072B which included restoring access road, restoring grade and applying erosion control measures where needed.
1682331793 2015-11-16 Plug / Plug Back No Violations On the day of my inspection I witnessed Wrights Well Service was onsite along with Appalachian Well Surveys and Formation Cementing Service. AWS tripped inside the 4.5 inch casing with a 4.5 inch casing cutter successfully shooting the casing off at 800 feet. WWS tripped 1 joint of casing from the wellbore and Formation gained circulation and pumped a 40 sack 200 foot Class A rip plug from 756 feet to 556 feet. Cement was displaced with 8.8bbls of water. WWS tripped casing to 200 feet and Formation will set the surface plug.
1878584317 2016-06-20 Final Restoration No Violations On the day of my inspection I met with Eclipse Resources EHS Superintendent Bruce Carpenter at the site to review restoration. The site now meets ORC 1509.072B with a land owner waiver for the access road left in place for farm use.
