0004211928 2015-11-02 Production Wells Violations Noted At 9:20 a.m. I received a call about a small spill from the Guernsey County inspector, Doug Bennett. He was aware that I was in the area of the Gulfport Miller 1H pad. At the time I arrived at 9:30 a.m. Gulfport and ECM were on location. There are 8 400 bbl. tanks within a dike. I was informed that at 3:30 a.m. an ECM water truck was pumping produced water off of the tanks. The sight glass on the back of the truck broke and 20 bbl. of produced water was spilled onto the ground outside of the dike. I witnessed Gulfport and ECM working together and hand dug small pits to gather the produced water and then to pump up the produced water from the pad. During my time on location there was a 130 bbl. ECM truck with fresh water and an empty 100 bbl. ECM truck to pump up the produced water out of the hand dug pits. Outside of the dike and beside the pad a small ditch had contamination within it. They are to remove the contamination and dispose of it properly. I left location at 11:20 when Trisha Cartmell arrived on location to assist with the inspection.
0035318283 2014-12-01 Completion Testing No Violations On the day of my inspection following up on NOV-1416154078, I witnessed that all contaminated stone has been removed and Gulfport is waiting on final lab results which are due on Tuesday 12/2/14. Upon receipt of good test results the stone will be replaced and primary containment relayed. Due to the extent of clean-up I granted an extension on NOV-1416154078.
0072230959 2014-11-24 Completion Testing No Violations On the day of my inspection following up on NOV-1416154078, I witnessed that the contaminated area has been cut down and tests provided acceptable chloride levels. Contaminated stone has been placed on secondary containment and is being loaded in roll off boxes and being properly disposed as boxes become available with remainder to be hauled off site by Tuesday with new stone being laid and compacted followed by new primary containment. SPSI continues to monitor area with levels being acceptable.

Annular pressure monitoring was present along with proper identification and production records being current.

0368672282 2015-03-16 Preliminary Restoration No Violations On the day of my inspection I witnessed that the well is online and equipped with a production tree with valves in the open position. Production equipment included a sand trap, a line heater, a vapor recovery system, 2 compressor units 2 separators, a combustor unit, a flare stack and a total of 8-400bbl tanks and 50bbl tank all within containment and shared with the 2-25H and 3-25H. A pig launcher is located on northwest corner of the pad. The site passed preliminary restoration according to ORC 1509.072A.
0580391301 2014-12-04 Completion Testing No Violations On the day of my inspection following up on NOV-1416154078, I received analytical test report on Tuesday and after reviewing the satisfactory results I gave the verbal okay to begin to replace stone with no further remediation to take place. Upon arrival I witnessed that clean-up was completed with the stone replaced and new containment being laid. I instructed operator to continue to monitor sumps for any possible migration. NOV-1416154078 has been resolved.
0604217213 2014-11-17 Completion Testing No Violations On the day of my inspection following up on NOV-1416154078, I witnessed that clean-up was continuing with 2500 gallons of acid recovered with the area outside primary containment being neutralized. Gulfport has begun removing equipment so that all secondary and then primary containment can be cleaned and neutralized before removing and disposal.
0697904537 2014-06-16 Casing String Witnessed No Violations On the day of my inspection I witnessed Nomac Drilling Rig #311 drilled a 12.25 inch diameter hole to the depth of 2200 feet and Wyoming Casing crew ran 2182 feet of 9.625 inch 36#/ft. J-55 Intermediate Casing. Halliburton pumped 25bbls of gel, 10bbls of fresh water and cemented casing with 730 sacks of class A cement. Displaced cement with 165bbls of water had good cement returns of 20bbls, plug was landed @980psi and pressured up to 1650psi for 5 minutes and the floats held. Cement stayed to surface.
0950748780 2014-11-18 Completion Testing No Violations On the day of my inspection following up on NOV-1416154078, accompanied by Southeast Regional Manager Rocky King, I witnessed that clean-up was continuing with equipment being moved on the Northeast corner of the pad so that secondary and primary containment could cleaned, neutralized and removed. The pad sumps have been pumped and continue to be monitored along with the pad for hot spots. Completed walk around with Roy Lee of Stingray and discussed continuation of clean-up and remediation.
0969457588 2015-02-20 Completion Testing No Violations On the day of my inspection I witnessed that flowback operations were ongoing and should be completed by next Wednesday. I met with Consultant Ross Satchel and completed walk around revealing proper containment, trucks displaying proper placards and no violations. At this time the pad is producing 2MMCF in gas per day per well, 1300bbls of oil and 450bbls of water total between the three wells 34059242230000 Miller 1H, 34059242910000 Miller 2-25H and 34059242900000 Miller 3-25H.

The production equipment included 8-400bbl tanks and 1-50bbl tank within containment, a vapor recovery tower, a vapor blow case, a 3-phase vertical separator, 2-compressor units, a vertical heater treater, a gas combustor and back-up flare stack all shared with the Miller 2-25H and Miller 3-25H. Also present a horizontal line heater, a vertical separator and a sand trap. The well was equipped with a production tree with valves in the open position with 350psi on the tubing and 500psi on the casing. Annular pressure monitoring was present along with proper identification and production records being current. Field tests conducted on the sump water revealed a pH of 6 and chlorides at 68ppm.

During my inspection the well was equipped with a plunger lift system with valves in the open position. The production casing and production tubing had separate production flow lines. Gauges were present on all casing strings. The 13.375-inch diameter production casing had 0 pounds per square inch, the 9.625-inch diameter intermediate casing had 100 pounds per square inch, the 5.5-inch diameter production casing had 400 pounds per square inch, and the production tubing had 350 pounds per square inch. The production equipment included eight 400 barrel tanks, one 50 barrel tank , a vapor recovery tower, a vapor blow case, a 3-phase separator, two compressors, a vertical heater treater, a gas combustor, back-up flare stack, a horizontal line heater, a vertical separator, and a sand trap.

1402495774 2014-06-11 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #311 was 95% rigged up with plans to spud this afternoon. I met with Company Men Jimmy Stewart and Chuck Restielle and discussed upcoming spud, drilling program and the north surface side of location that is being address with Gulfport construction.
1402624732 2014-06-12 Casing String Witnessed No Violations On the day of my inspection I witnessed that Nomac Drilling Rig #311 drilled a 17.5 inch diameter hole to the depth of 400 feet. While tripping out of the hole Nomac Drilling Rig #311 hit a tight spot in the borehole at 325 feet and continued out of the hole and tripped back to ream to bottom. Nomac Drilling Rig #311 then had mechanical issues with the vee-door lift which had to be repaired. Wyoming Casing Crew rigged up and ran 403.75 feet of 13.375 inch 54.5#/ft. J-55 casing the last 10 feet had to be washed down. Halliburton was on site and gained circulation with 10bbls of fresh water then pumped 25bbls of gel, 5bbls of fresh water and cemented casing with 470sks of Class A cement displaced cement with 56bbls of fresh water. Good returns of 12bbls of cement, plug was landed at 136psi and pressured up to 640psi for 5 minutes and the floats held. Cement stayed to surface.
1416154078 2014-11-16 Completion Testing Violations Noted On the day of my inspection I was notified by Kris Anderson of All Consulting that a Stingray tanker released of 4006 gallons of 32% HCL which had spilled onto secondary containment overflowing to primary containment. Upon arrival I met with Belo Kellam Completions Supervisor and he reported that 4006 gallons of 32% HCL has released into secondary containment which overflowed onto primary containment which was not bermed due to moving in of equipment. Some acid has reached the stone on the pad which was resulting in a ploom blowing north east off of the pad. The area had been diked with sand and lime to contain. There was a voluntary evacuation of 2 homes on Jasper road at the time of inspection. Antrim Fire Department was called and on site for emergency evacuation. Specialized Professional Services Inc. was onsite and accessed tanker finding a 3 inch diameter breech in the weld on the belly with approximately 50 gallons of acid remaining. SPSI transferred free standing acid into a new tanker. 1800 gallons was recovered as SPSI was pooling remainder inside secondary containment. Primary containment was going to be pooled and pumped into new tanker along with neutralization of area outside primary containment and removal and replacement of primary and secondary containments. Owner/Operator shall continue to pump up acid inside containment. Upon completion neutralize containments before removing and replacing. The pad shall be monitored for hot spots and neutralized along with continued monitoring of pad sumps.

1446566565 2015-11-03 Production Wells No Violations My inspection today was to follow up on a compliance notice #4211928. 20 bbl. of produced water was spilled onto the pad right outside of the dike when the trucks sight glass broke. The pad contamination outside of the dike has been flushed with fresh water and the contaminated soil has been removed. The soil has been placed in a roll off container and will be taken to an approved land fill. All issues have been resolved.
1449158907 2014-11-19 Completion Testing No Violations On the day of my inspection following up on NOV-1416154078, I witnessed that clean-up was continuing with the sand silos being emptied. All equipment has been moved from the wellheads to the sand silos. A grid has been laid out and testing has begun on area where primary and secondary containment was removed. Accompanying Stingray Representative Roy Lee and SPSI Supervisor Lance Wilcox along with SPSI associates I witnessed SPSI excavate every 10 feet 6 to 8 inches deep within the grid revealing normal PH levels. Areas that were outside primary containment and affected were also excavated revealing normal PH levels. Further testing will be completed upon removal of the sand silos and tanker used to recover acid. Upon completion of testing SPSI and Gulfport will furnish detailed report to the Division.
1479456032 2014-11-20 Completion Testing No Violations On the day of my inspection following up on NOV-1416154078, I witnessed that the silos and tanker have been removed and during walk around I found utilizing PH strips and Quantabs that the PH were normal levels but the Chlorides were ranging from 4500ppm to 6000ppm in the affected area. After conferring with Gulfport officials and lengthy discussions SPSI was brought in to take soil samples for a baseline. Then the decision was made to begin to excavate the affected area until chlorides are brought into an acceptable level. Upon satisfactory results the stone will be replaced.

1558905341 2015-06-19 Production Wells No Violations On the day of my inspection accompanied by Regional Emergency Operations Manager Trisha Cartmell, we reviewed the incident that occurred on November 16, 2014 where a Stingray tanker released of 4006 gallons of 32% HCL which had spilled onto secondary containment overflowing to primary containment resulting in zero injuries. Casing pressure was at 1550psi and tubing pressure was at 1350psi with valves in the open position.
