0198572693 2018-03-20 Production Wells No Violations On the day of my inspection accompanied by ODNR Inspector Jeff Adams, I witnessed the production equipment included a 210 barrel tank within an earthen dike. Also present a horizontal separator, a drip, a motor valve, domestic gas service, a 2 inch meter run with 1 inch orifice and a chart recorder. The well was equipped with 8.625 diameter casing, 4.5 inch diameter, 2.375 inch diameter tubing and a plunger lift system with valves in the open position on the tubing and 1 of 2 open on the casing. Gauges present read 700 pounds per square inch on the casing and 490 pounds per square inch on the tubing. Identification was present and production records were current.
0440161141 2009-06-26 Drill / Deepen / Reopen Violations Noted Pit leak which leaked water, gell and cement through crack in rock ~ 100 below location. Fluids ran ~ 500 downhill (though most were soaked into the soil) and reach an unnamed tributary of the Wills Creek. No fish kill observed. Cl- were 13000. Mostly from the potash that was in the soap water that hey drill the surface hole with.
