Note: Called and discussed area north of wellhead it appears that when site was restored this area was left low allowing water to stand,field supervisor stated that as soon as weather permitted they would apply more top soil and seed&mulch area too allow water to drain off site properly.

1322149257 2018-03-21 Production Wells No Violations On the day of my inspection accompanied by ODNR Inspector Jeff Adams with the ground snow covered, I witnessed the production equipment included a 210 barrel tank within an earthen dike. Also present a horizontal separator, a drip, a motor valve, domestic gas service, a 2 inch meter run with 1 inch orifice and a chart recorder. The well was equipped with 8.625 diameter casing, 4.5 inch diameter, 2.375 inch diameter tubing and a plunger lift system with valves in the open position on the tubing and 1 of 2 open on the casing. Gauge present read 480 pounds per square inch on the casing. Identification was present and production records were current.
