Howard drilling had Appalachian Wireline on site. The rig pulled approximately 24 inches of stretch. The 4---0 feet. The Compliance Notice number -1126374531 and Complaint number 5507 have been resolved due to the well being plugged.

Brent Bear and I were on site and revealed that Howard Drilling was going in and place the large plug from 5145-4345. The large plug was placed due to not being able to get down hole. The ran 228 sacks of regular class A cement and displaced down with 13.5 barrels of displacement thru the split string. The rig shut down for day after pulling the tubing and up and out of the cement. Tag the cement plug in morning.

1126374531 2017-01-17 Production Wells Violations Noted On the day of my inspection following up on Complaint ID 5507, I witnessed the production equipment included a 210 barrel tank, a 100 barrel tank, a horizontal separator, a drip, a 2 inch salesmeter with a .5 inch orifice and a chart recorder all within an earthen dike and the chart present dated 2/8/12. The lease road was heavily vegetated with downed trees. The well was found in heavy vegetation and equipped with 4.5 inch diameter casing; 2.375 inch diameter tubing and a plunger lift system with all valves in the closed position on the tubing and 1 of 2 valves on the casing open and controlled with a .25 inch needle valve found in the closed position. A light smell of gas was detected near the wellhead. Readings were taken with ODNR Honeywell PhD6 meter revealing a LEL of 4 and no H2S detected. Also utilizing ODNR Tango H2S detector revealed no H2S detected. Present near the wellhead a damaged windsock and lying on the ground within heavy vegetation a Caution Hazard Area Hydrogen Sulfide (H2S) sign was found. Identification was present but faded and production records were current with no production reported in 2012 and 2013 and zero production reported in 2014 and 2015. The well owner/operator shall submit a written plugging schedule to Doug Bennett at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.

The compliance notice stated that the well shall be plugged. During my inspection the well was equipped with 4.5-inch diameter production casing with a flange tubing hanger tubing head, 2.375-inch diameter tubing, and a plunger lift system. All valves were in the closed position. The production equipment consisted of one 210-barrel tank, one 100-barrel tank, and one horizontal separator.
1544472629 2018-12-07 Plug / Plug Back No Violations The inspection is conducted under plugging operations and a Compliance Notice follow-up to number -1126374531 and complaint number 5507. Howard Drilling ran in the well and tagged the 800- foot plug at 4550 feet. The 2 3/8- inch tubing is being ran in the well to set the Big Lime plug from 3850-- inch tubing. The plug is being placed thru the tubing string at the perforations at 3850 feet.
1547137595 2018-12-04 Plug / Plug Back No Violations The inspection is conducted under plugging operations and a Compliance Notice follow-up to number -1126374531 and complaint number 5507. Howard drilling had all the 2 3/8 inch upset tubing pulled out. There was a roller bit on the bottom of the tubing string. Appalachian wireline was on location running a bond log. The cement records from the completion records show that they ran 90 sacks of cement on the long string. The bond log shows that the cement top is at 7050. There were probably 190 sacks ran on this job or 800 feet of fill-up. The bond log shows tight spots in section up hole. From 4200-3950, 4600-4500, 3190-2750 and 2550-2500 feet. The decision was made to run in and perforate with .49 shots over a 1 foot per 10-foot section. 1st shot was at 6890-6880 feet for the Trenton plug, 2nd shot was at 5250-5240 feet for the Clinton plug. The 3rd shot was placed at 3850-3840 feet for the Oriskany / Big lime plug. There was a tight spot at about 4800 feet and this would be at the Newburg section. Appalachian wireline had to use a sinker bar to get the shots placed at the intervals. Howard drilling was going to run 2 3/8 inch upset tubing back in the well to spot the plugs.

During my inspection the location was rough graded with no vegetation growth. The production equipment had been removed with the exception of one cylindrical horizontal tank. The location remains within the timeline given for final restoration requirements.
