0268397332 2018-07-24 Final Restoration No Violations The purpose of my inspection was a status check on oil contamination. During my inspection there had not been any change since my last inspection, the production equipment and the oil contamination remains. Oil surrounds the 210-barrel tank in 30 feet by 30 feet area within the dike. A 20 inch by 10 feet path of oil was on the outside of the dike along with stressed vegetation. Green Gas will be contacted regarding the contamination and production equipment. As stated in 1509.072(B), to reach final restoration requirements within six months after the well is plugged the owner shall remove all production and storage structures, supplies, and equipment, and any oil, salt water, and debris, and fill any remaining excavations. Within that period the owner or the owners agent shall grade or terrace and plant, seed, or sod the area disturbed where necessary to bind the soil and prevent substantial erosion and sedimentation.
0485664000 2019-04-15 Final Restoration No Violations The purpose of my inspection was a follow up on compliance notice -1390810308. The compliance notice was written for failure to complete the final restoration within six months after plugging and abandoning. The compliance notice required that the remedy work be completed by 4/11/2019. During my inspection production equipment and oil contamination remained on location. The land owner had removed the obstructions that he owned from the lease road leaving an open road to the tank battery. Scott Davis with Green Gas was made aware of the work that needs completed.

1247224584 2018-09-27 Final Restoration No Violations Accompanied by ODNR SouthEast regional manager Brent Bear. During our inspection we found oil contamination still present within the dike beside the 210 barrel tank. The 100 barrel tank was evaluated and did not appear to be leaking. A horizontal seperator was found within thick vegitation. The lease road was walked back to the wellsite and the wellsite was evaluated with vegitation established. Residence were either not home or did not come to the door.

The 210 barrel tank appears to be leaking from the bottom and a 30 feet by 30 feet area of oil contamination was present surrounding the tank inside the dike. The 210 barrel tank appears to be leaking from the bottom and a 30 foot by 30 feet area of oil contamination was present surrounding the tank inside the dike. Also present an 18 inch by 10 feet area of oil contamination with stressed vegetation was found outside the dike. Identification was on the 210 barrel tank but was illegible. Land owner Sam Bradshaw contacted me this morning and stated that he never signed a landowner waiver and that Green Gas was supposed to remove the equipment two months after plugging was completed.
1390810308 2019-03-20 Final Restoration Violations Noted The purpose of my inspection was to follow up on complaint 5557 regarding oil contamination. On 2/4/2019 I spoke with land owner, Sam Bradshaw, about the production equipment in the back yard of his residence at 55982 North Star Road in Pleasant City belonging to the well named the Bradshaw unit, permit number 34059237090000, owned by Green Gas. Mr. Bradshaw stated that he did not sign a land owner waiver (Form 5) to allow Green Gas to leave the production equipment on his land and that he would like to see the oil get cleaned up. The well 34059237090000 was plugged on 8/10/2006 and the production equipment was not removed from the site. During my inspection today, the production equipment left on site consisted of one 210-barrel tank, one 100-barrel tank, a drip, a horizontal separator, a sales meter with a chart recorder, and two electric utility poles. An area 30 feet by 30 feet within the dike and an area 18 inches wide by 10 feet long outside of the dike was impacted by oil contamination. The oil appeared to have been released from the 210-barrel tank. I called Scott Davis with Green Gas on 3/19/2019 to ask if any land owner waivers (Form 5) were signed to gain permission to leave the production equipment on site and he replied that there were no waivers signed but the tanks were left there by the instruction of the land owner. In order to comply with 1509.072 (B), Green Gas Company shall remove all production equipment, and any oil, salt water, and debris, that was associated with 34059237090000 the Bradshaw Unit. Following the removal of the production equipment and removal of the contamination, all excavations shall be filled, and the disturbed area shall be graded and seeded to establish vegetation.
