0170742544 2017-07-26 Production Wells No Violations On the day of my inspection I following up on Compliance Notice 432762577, I met with Deer Creek personnel Dave Williams and Susan Bates to discuss the notice. The contamination from the wellhead needs to be removed, remediated and replaced with clean fill. The contaminated soils need to be disposed of in an approved landfill and manifest need to be provided to ODNR upon disposal along with the well identification updated. If they intend to produce fluids near the wellsite a dike needs to be constructed with a proper storage tank.
0432762577 2017-07-20 Production Wells Violations Noted On the day of my inspection I witnessed the production equipment included 2-horizontal separators, a vertical separator and a drip. Also present 2-210 barrel tanks and a 50 barrel tank within an earthen dike. All equipment appears to be tied in and shared with 34059211980000 Mather-Forest A-1. A 3 feet by 4 feet area of oil contamination was present near the 3 inch load valve on the south tank. Also within the dike a 4 feet by 20 feet area saturated with standing brine was found. Fluid was field tested and revealed a pH of 6 and chloride levels of > 6215 parts per million. The well was equipped with 5.5 inch diameter casing, 2.375 inch diameter tubing, rods and a pumping unit powered by electricity with 1 of 2 valves open on the casing routed to the separator. 1 of 2 valves was open on the tubing reduced to a 1 inch diameter plastic line routed to a portable 13 barrel steel rectangular tank. Also present a disconnected 210 barrel tank. A 13 feet by 130 feet area of oil & brine contamination was found at the base of the portable tank which was set 18 feet from the wellhead and had no plug in the drain line. Identification was present by not legible and production records were current. Owner/Operator shall properly identify well in accordance with OAC 1501:9-9-05 (A) (10). Owner/Operator shall remove fluids from dike, remove contaminated soils and replace with clean fill. If Owner/Operator elects to keep the 13 barrel portable tank near the well, the tank shall meet proper setback distance from the well in accordance with OAC 1501:9-9-05 (A) (2) and a SPCC dike shall be constructed in accordance with OAC 1501:9-9-05 (A) (9). Owner/Operator shall remove contaminated soils at the wellsite, replace with clean fill, dispose of all soils in a solids landfill and provide ODNR with the manifest for the disposal.

The contamination has been removed, remediated and placed into a Buckey Disposal roll of box for proper disposal. Contaminated soils will be disposed of in a solids landfill and the manifest will be provided to ODNR upon disposal.
