0568954274 2017-03-08 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with 4.5 inch diameter casing, 2.875 inch diameter tubing, 1.5 inch diameter tubing, a plunger lift system and domestic gas service with valves closed on the 1.5 inch diameter tubing and 1 of 2 open on the 2.875 inch diameter tubing supplying the domestic gas service. The production equipment included a 100 barrel tank, a horizontal separator, a 2 inch salesmeter with .75 inch orifice and a chart recorder with no chart present all within an earthen dike. Identification was present and production records were current.

1549473424 2019-02-06 Production Wells Violations Noted The purpose of my inspection was a follow up on complaint number 6068. The complainant stated that the well had been abandon for more than a decade. Inspection found 4.5-inch diameter production casing with a casing head, 2.875-inch diameter intermediate casing with a tubing head, 1.5-inch diameter production tubing, and a plunger lift system. Production valves appeared to be in the closed position. The 2.875-inch intermediate casing had a domestic gas service attached with a Little Joe Regulator and an open valve. I contacted the landowner Tim Lasko and asked him if the well was providing domestic gas for any local residences and Mr. Lasko stated that the domestic line was shut off at his house and it was not providing domestic gas. The production equipment consisted of one 100-barrel tank, one horizontal separator, and a sales meter. The sales meter had a 2-inch diameter sales line, a .75-inch orifice plate, and a chart recorder with a chart that was not legible. The production reports that have been submitted to ODNR report records of zero production from years 2007 to 2017. I left a voicemail with Georgetown Oil Company with call back information. Georgetown Oil Company shall submit a written plugging schedule to Austin Gutridge at 5880 Memory Rd., Zanesville Ohio 43701 within 30 days, per ORC 1509:13(B)(7) and plug the well within 180 days. Georgetown Oil Company will be notified if the plugging schedule is approved or denied.
