0368968697 2016-06-08 Final Restoration No Violations On the day of my inspection at 4:10pm I received a call from Artex Superintendent Joe Snider stating that there was a leak in the dike at the site. Upon arrival at 5:10pm I witnessed a 300bbl tank within an earthen dike. The well had received final restoration status on 2/21/08. The dike was saturated with fluid inside the dike approximately 4 feet around the tank and area of 6 inches by 10 foot outside the dike. A 3 inch valve not plugged was in the closed position with cattle tracks surrounding the tank. 50 feet away from the tank a deceased bull was found. J Hawk Services was onsite and began removing fluid from inside the dike retrieving 3bbls. J Hawk then pulled remaining fluid from the tank. Artex employees applied absorbent peat and bio sorb around the tank. Bandit Energy Services will be onsite tomorrow to remove the tank and complete remediation of site.
