0210904434 2017-08-15 Plug / Plug Back No Violations On the day of my inspection I witnessed Miley Gas Co. (MGC) had the pit dug and lined with plastic to the wellhead. Appalachian Well Surveys (AWS) arrived and tripped inside the 5.5 inch diameter casing finding water level at 70 feet and tagging the bottom hole plug that was set on 8/25/16 at 465 feet. AWS completed bond log of the 5.5 inch diameter casing revealing it was free at 450 feet. AWS tripped out of the 5.5 inch diameter casing and tripped back in with a 5.5 inch casing cutter and successfully shot the casing off at 430 feet. AWS tripped out and rigged down. MGC successfully freed up casing and began laying it down. Upon completion AWS will return and perforate casing for plug completion tomorrow.
0627096428 2017-08-16 Plug / Plug Back No Violations On the day of my inspection I witnessed Miley Gas Co. (MGC) was onsite and Appalachian Well Surveys (AWS) had perforated the 7 inch diameter surface casing at 380 feet with 4 shots and at 130 feet with 4 shots. MGC tripped 2 inch diameter tubing to 430 feet. Formation Cementing Inc. arrived and pumped a 25 sack 150 foot Class A Bottom of the surface plug from 430 feet to 280 feet. Cement was displaced with .75 barrels of fresh water. MGC tripped 12 joints of 2 inch diameter tubing from the wellbore and Formation Cementing Inc. pumped a 42 sack 160 foot Class A Surface plug from 160 feet to surface circulating cement to surface. MGC tripped remaining tubing from wellbore and cement remained at surface. Instructed operator to cut casing off 30 inches below grade and install metal plate identifying well and reclaim according to ORC 1509.072B.
0883706638 2016-03-07 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with 6.625 inch casing, 2 inch tubing and rods with valves in the closed position. Production equipment included 2-100bbl tanks shared with 20711, 20602 and 22381 within an earthen dike. Jeff Miley of Miley Gas Co. claims he is the owner of the said well and is trying to get it transferred into his name so that it can be plugged.
1472133510 2016-08-25 Plug / Plug Back No Violations On the day of my inspection I witnessed Miley Gas Co. (MGC) had tripped 2 inch tubing into 654 feet. Cemenco Services Inc. was onsite broke circulation, pumped a bottoms up, pumped a 5 barrel gel spacer and then pumped a 180 foot 20 sack Class A bottom hole cement plug from 658 feet to 478 feet. Cement was displaced with 1.8 barrels of fresh water. MGC tripped 2 inch tubing out and packed off wellhead. MGC will come back along with Appalachian Well Surveys to do a bond log on the 5.5 inch casing, shoot the casing and complete plugging operations.
1598540018 2017-08-07 Plug / Plug Back No Violations On the day of my inspection I witnessed Miley Gas Co. (MGC) had not arrived to complete the plugging of the said well. At 13:16 I received a call from MGC Jeff Miley stating that he is planning on bringing the rig over in this week and finding free point on the 5.5 inch diameter casing and having Appalachian well surveys cut casing.
1669389295 2016-08-18 Production Wells No Violations On the day of my inspection I witnessed Miley Gas Co. (MGC) trip rods out of the wellbore in preparation of upcoming plugging. Appalachian Well Surveys (AWS) was onsite for the perforation of the 2 inch tubing. AWS was unable to get past 200 feet and rigged down. MGC will trip tubing from wellbore tomorrow.
1749533920 2016-03-29 Production Wells No Violations On the day of my inspection I met with Muskingum County Watershed District Representatives Mark Swiger, John Watkins and Brad Janssen along with Miley Gas Co. Owner Jeff Miley to discuss upcoming plugging of well, pit placement and erosion control measures for the location.

2003565894 2016-08-08 Production Wells No Violations On the day of my inspection I met with Muskingum County Watershed District Representative Brad Janssen along with Miley Gas Co. Owner Jeff Miley to review the proposal for plugging of the well.
2072144142 2016-03-01 Administrative Inspection No Violations I met with Ruth Burns at her home on route 566 to discuss the well. Ms. Burns stated that she believed the well was sold to Jeff Miley of Miley Gas Company. I contacted Mr. Miley and he indicated that he did own the well and he is in the process of registering the well in his name. Mr. Miley also stated that he intends to plug the well this summer.
2100224481 2017-08-14 Plug / Plug Back No Violations On the day of my inspection I witnessed Miley Gas Co. (MGC) had tripped 2 inch tubing from wellbore, removed wellhead and cut 7 inch diameter casing off below grade to get their elevators on the 5.5 inch diameter casing. Appalachian Well Surveys will be onsite tomorrow to find casing free point and cut casing.
