1295442597 2011-01-18 Administrative Inspection No Violations Got call from Sheriff - strong gas odor filling entire valley. Reportedly, in area of Lost Rd. Investigated, calling Dave Hill and Enervest after determining they owned wells in the area. Dave Hill responded quickly, did not find gas odor in area of Lost Rd, but did find moderate, dissipating odor east of interstate at Byesville. After finding no problems or leaks, determined that pumper had blown off a drip or possibly just well running and sending gas to the tank, were the cause of the problem, combined with fact that air was VERY heavy and gas was laying low to the ground. Called Petrox to make them aware of the complaint, as they had wells in the area also. Called Sheriff and Alice and told them of our findings.
1413254871 2018-03-13 Production Wells No Violations On the day of my inspection I witnessed the production equipment included a 210 barrel tank, a horizontal separator, a drip, a 2 inch meter run with .25 inch orifice and a chart recorder all within an earthen dike. The well was equipped with 8.625 inch diameter casing; 4.5 inch diameter casing, 2.375 inch diameter tubing, rods and a Lufkin pumping unit powered by natural gas with 1 of 2 valves in the open position on the tubing and valves in the open position on the casing. The belts were broke on the unit and the power was in the off position. Identification was present and production records were current.
