Identification was present with contamination still present inside the dike. The chart was changed on 8/6/15 showing the well is cycling which is the Jenkins LePage #7 34059216800000 but has the Jenkins LePage 6 and 7 written on the location on the chart. There was no change in the status of the said well since last inspection and field testing of water in the dike revealed <30ppm chlorides and a pH of 6.

There was no change in the status of the said well since last inspection and updated photographs were taken.

G an R shut his house gas off at that time. I did my inspection of the well and the location and all the material is still on the well site.

1377007275 2013-08-20 Production Wells Violations Noted On the day of my inspection I meet with the land owner Tom Henry, At the well site I witnessed four and a half inch production casing with a (4x2) swedge and valve standing from the ground, the valve on top of the (4x2) swedge was open, all production lines and sales lines were disconnected from the well, the pumping unit has been pulled away from the well but is still on location, the tubing and rods have been removed from the well and are on location, the previous well owner told me the casing parted down hole approximately three or more years ago. Production has been reported to The Division of Oil and Gas from the year of 1984 to 2012. There are (2) two hundred ten barrel production tanks/ separator/ drip and chart setting inside a containment dike, there is containment soil around the tanks, the I.D. is posted on one of the tanks. The requirements to meet the Division of Oil and Gas Standards will be to make the well capable of commercial production within thirty days or plugging is required within thirty days. Failure to comply may result in Forfeiture of Bond.
