During my inspection the well was equipped with 8.625 inch diameter surface casing with a casing head, 4.5 inch diameter production casing with a 4.5 inch to 2 inch diameter swage nipple. Production valves were in the open position. The production equipment consisted of one 100 barrel tank. Identification was present and production records were current.

1553863946 2019-03-28 Production Wells No Violations The purpose of my inspection was a status check. During my inspection the well was equipped with 8.625-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a 4.5-inch to 2-inch diameter swage nipple. Production valves appeared to be in the closed position. The production equipment consisted of one 100-barrel tank and a 50-barrel poly tank. Identification was present and production records were current with M&R Investments submitting records of gas production.
1993269373 2014-12-23 Production Wells No Violations On the day of my inspection I received a phone call from Eclipse Representative Bruce Carpenter stating that a gas line had been hit on 57275 Claysville Rd. Upon arrival I witnessed that New Concord Fire Department and EMS were on site and a 4 inch fiberglass line had be hit by Strauss Fencing Employees Toby Knipe and Devin Angelo both receiving non-life threating injuries and being released by EMS. The 4 inch line was from an Eclipse Compressor Station located on Claysville Rd with no tracer wire present. The gas had been shut off and Eclipse employee Jerry Love was making repairs to the 4 inch line with no contamination present. The line was identified approximately 150 feet south of the property by an Eclipse marker with no marker present on said property. The said well was a swab well and equipped with 8.625 inch casing, 4.5 inch casing 2 inch tubing with valves in the open position. Production equipment included a 100bbl tank and a 13bbl brine tank with the salesline leading to the compressor located on Claysville Rd.
