0210544537 2018-07-12 Final Restoration No Violations The purpose of my inspection was a final restoration. During my inspection all restoration work has been completed. One 100-barrel production tank remains. Once the tank has been removed the location will meet the final restoration requirements.
0242837589 2016-03-07 Production Wells No Violations On the day of my inspection I witnessed the well was equipped with 8 inch casing 6.625 inch casing, 2 inch tubing and rods with valves in the closed position. Production equipment included 2-100bbl tanks within an earthen dike. Jeff Miley of Miley Gas Co. claims he is the owner of the said well and is trying to get it transferred into his name so that it can be plugged.
0961032370 2018-05-21 Plug / Plug Back No Violations The purpose of my inspection was a status check on plugging operations. During my inspection Jeff Miley had his single pole service rig parked at the entry of the lease road. Mr. Miley was clearing brush and trees out in order to gain access to plug the well.
1321248830 2018-10-03 Final Restoration No Violations The purpose of my inspection was a status check on a final restoration During my inspection thick vegetation was in place and all production equipment has been removed. The location meets final restoration requirements written within the Ohio Revised Code 1509.072(B). A gate has been placed at the old lease road entry by the Muskingum Watershed Conservancy District to restrict Seneca Lake access from that location.
1526488111 2018-05-14 Production Wells No Violations During my inspection I met with Muskingum County Watershed District Representatives Mark Swiger and John Watkins along with Jeff Miley the owner of Miley Gas. The meeting was held at the well. The purpose of the meeting was to go through the plugging procedure and to talk about a restoration plan. Mr. Miley stated that he would like to get the rig moved in this week and hopefully have the well plugged by Memorial Day weekend.

Owner Jeff Miley to discuss upcoming plugging of well, pit placement and erosion control measures for the location.
2046986025 2018-06-05 Plug / Plug Back No Violations The purpose of my inspection was to check that the surface plug pumped on 5/24/18 did not descend down hole. During my inspection the surface casing had not yet been cut 30 inches below surface grade. The cement plug stayed at surface and is visible inside the surface casing. I will confirm with Miley Gas that I have checked the surface plug and they are now free to cut the casing down 30 inches below surface grade, install the steel identification plate, and proceed with restoration requirements written under 1509.072 (B).
