0131906849 2013-10-08 UrbanProduction Wells No Violations My inspection found that the company has planted 15 pine trees on the location to help block the well and the tank battery from the surrounding houses. Vegetation has been established through out the location. Nothing else has changed since my last inspection. Preliminary restoration is passed.
0196249876 2011-12-15 Urban Drill / Deepen / Reopen No Violations Drilling operator contacted me about gas encountered during the drilling of the surface hole. While I was on site, the operator noted gas after tripping first section of drill pipe out of hole. Hole was gassing at the time of inspection and while driller was running surface casing. Gas was blowing fluid 1 to 2 out of the hole when casing was tripped in. There was compression of gas inside of the casing while it was being installed. Once the pressure was released the gas was still noted venting from casing, but at a decreased rate. Driller completed running surface casing, broke circulation and circulated bottoms up three times to circulate gas kick out of the hole. After discussion with Roy Poulson, Tim Cosmo (Cedar Valley), Petroset Cementing, Rick Simmers (Chief of the Division of Oil and Gas) and Bill Bennet (Cedar Valley) it was decided to circulate bottoms up with cement to circulate gas out and kill source of gas. It was agreed, if there was any indication of gas cut cement the operator would circulate cement out of the hole and continue to condition the hole. Petroset Cementing broke circulation, ran 300 lbs of gel, 2 bags of hulls and then changed over to run cement. They ran 150 sacks of Light cement ahead to circulate any remaining gas out of the hole. Cement returned to surface and smoothed out, indicating gassing had subsided. They then ran 150 sacks of Class A with 3% CaCl. Cement weight during mixing of light cement was 15 lb per gallon, 14 lb per gallon and 15 lb per gallon. Petroset mixed light cement heavy to decrease set time. Calculating a yield of 1.18 for class A cement, 348% excess cement was run for the job. The cement fell back 3 to 4 after displacement and the operator bailed cement from the ditch into the annulus space between conductor and surface casing. The cement stabilized and held at surface. We waited for one hour and the cement was static. We took air readings during event with a Sperian Gas Detector. Readings w
0198894302 2013-06-14 UrbanProduction Wells No Violations My inspection today is to follow up on the preliminary restoration issues. Cedar Valley has not planted the pine trees requested in the pre-site agreement. The pile of stumps and trees that needed to be removed is still on location. I left a message with Tim Cosmo to tell me when the work will be preformed on the location.
0245696972 2013-04-18 Production Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valve was open and plumbed into a gauge and a pop off valve, which was then plumbed into the flow line. The annulus pressure was at 52 psi. The well and tank battery are surrounded by locked chain link fences. The well and tank battery are in good repair. The tank battery consists of a 100 BBL production tank, a 50 BBL brine tank, and a mechanical separator. The tanks had spark arrestors on them and were vented into charcoal filters. The load lines on the tanks had bull plugs in them and were locked. The proper identification was located on the tank battery and the entrance to the road. The lease road was in stable condition and was being used for a residential drive.
0579828326 2011-12-14 Drill / Deepen / Reopen No Violations On site for the cementing of the conductor pipe. The drift was 24 thick. Hole #1 diameter was 14.75 and total depth was 62. Conductor pipe was new 11.75, set at 52.50. The cement water pH was 8. The cement operator broke circulation, mixed cement with 3/4 pound of flak per sack, then displaced cement until it returned to surface. The cement held at surface and it did not wash the cellar out. The drilling of conductor created a wash area under the rig and the excess cement was left in place to stabilize the area.
0608088121 2013-06-24 Preliminary Restoration No Violations My inspection today found the brush pile on the west side of the lease road had been taken apart. The trees had been cut to length and stacked in piles for the landowners use. The tree limbs are in a pile on the west side of the lease road. The tree stumps had all been cut off the trees and stacked upright around the location. The pine trees will be planted in the fall. The site has been graded. I will wait until the pine trees are planted and the vegetation has been established before passing the preliminary restoration.
0961733881 2013-05-07 UrbanProduction Wells No Violations Met with Tim Cosmo from Cedar Valley about the preliminary reclamation of the location. I advised him to look over the pre-site requirements that were agreed upon. He stated he would take the stumps off of the trees and set them upright around the location so they can decompose better. He stated he would have the larger trees cut to 9 lengths, so the landowner can manage them better. The brush pile on the west side of the location will be moved and the larger trees will be taken out and cut into shorter lengths. Nothing else has changed since my last inspection.

The well had a domestic line with two regulators. The well is fenced and man door locked with proper and legible identification. The tank battery consisted of one, 100 BBL production tank, one, 50 BBL steel brine tank, and a mechanical separator. The tank battery was diked and had proper, legible identification. The tank battery was fenced and the man door locked. Lightening protection and charcoal filters were installed on the production tank. Lease access was stable. No violations.

The pits are in good repair with 3 or more of free board. The operator is collecting cutting samples every 10. Identification sign and safety signs posted at the entrance of the site. Lease road is constructed out of rock. Safety fence has been installed. Permit is posted in the dog house.
1324322362 2011-12-19 Urban Drill / Deepen / Reopen No Violations On site for the testing of the Blowout Preventor (BOP). The operator closed the BOP at 500 PSI, pressured up on the BOP to 250 PSI with a zero loss of pressure after 15 minutes. The pits were in good repair. The lease road was stable and the operator had topped the front of the lease road with additional stone. The charge bottle on the rig had 800 PSI on it and the back up bottle is on the rig. Two fire extinguishers where on the boom and one in the dog house. Permit was posted in the dog house.
1325178542 2011-12-23 Drill / Deepen / Reopen No Violations I was present for the cementing of the production casing. The cement operator broke circulation, ran the sweep and Dow Flak ahead of the cement, mixed and ran cement and locked the plug down. There was fluctuating pressure (300 PSI to 0 PSI) at the start of cementing. Circulation at the top of the hole reacted to this pressure fluctuation. The pump pressure eventually stabilized and circulation was constant during pumping and displacement of the cement.
1325708111 2012-01-04 Urban Drill / Deepen / Reopen No Violations The well owner solidified the pits using 4 loads of dust and then closed them. Frac tanks have been parked on top of the solidified pits. The annulus pressure on the 8.625 casing was zero. Tim Cosmo called and stated the 5 day test showed zero annulus pressure. Drainage controls were in place. Lease road was stable. Site was frozen.

The brush pile on the west side of the lease road needed pulled apart. The stumps cut off and set upright around the location. The tree limbs stacked in a brush pile. The trees needed cut and stacked for the landowners use.

The tank battery consisted of one 100 BBL tank, one 50 BBL tank, and a mechanical separator. Valves on the separator were open. There was a Dominion sales line within the tank battery. The urban safety equipment was installed on the 100 BBL tank. The tank battery was diked and fenced with a locked man door. Signage was located at the well and tank battery and the lease access was stable. No violations observed.
