0918563552 2013-07-25 Production Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The well and the tank battery are in good repair. The tank battery consists of a 210 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The proper identification is located on the tank battery. The lease road to the tank battery is in stable condition. The path back to the wellhead has washed out in numerous locations with deep ruts made from the landowners tractor.
1136101987 2011-12-28 Drill / Deepen / Reopen No Violations I was on site for the cementing of the surface casing. During the drilling of conductor hole the operator did not encounter the bedrock necessary to land the pipe. At 150 the operator was advised to drill with caution and contact me when bedrock was encountered. Bedrock was encountered at 190 (the permit called for 210). I reviewed the well logs on Clark Road, Arbor Glen Drive and Maple Glen Drive, for formation depths. Theres a significant change in geology going west to east on Arbor Glen. The Berea top and bottom - addresss on Arbor Glen: (11830) - Gray Sandstone 79-81, (11800) - Gray Sandstone Berea 53-108, (11815) - Gray Sandstone Berea 67-92, (11835) - Berea top 86 bottom 90, (11840) - Gray dry gravel/sand/clay/ mixed sandstone 52-79 blue wet shale 79-125 Black shale 125-146, (11855) - Blue gravel/clay/blue sand 15-158, adjacent well - blue gravel/ clay/blue/shells lime sand/ gravel 15- 158. Wells on Maple Glenn: (8715) - Berea 60-76, (8720) - Sandstone 76-98. The operator drilled the conductor hole with a 15 bit and continued using this bit to the todal depth of the surface hole at 230. The 8.625 surface casing was set at 211 (Ground Level). Cement water pH was 7. The cement operator broke circulation, ran gel and one bag of hulls ahead, mixed cement and displaced it with 12 barrels of fresh water. Good cement returns and the cement held at surface.
1325269130 2011-12-30 Drill / Deepen / Reopen No Violations Once the rig drilled to the top of the lime they loaded the hole and shut down for the weekend. Drilling will resume Monday morning. The Blow Out Preventer and air head were in place. The pits are in good repair with 2 or more of free board. I requested the the driller dam off the flow ditch from the cellar to the work pit. There are significant slopes associated with this site so drainage will need to be monitored on site and controls added as needed. The lease road was stable.
1350416487 2012-10-16 Preliminary Restoration No Violations I inspected this unit pursuant to a landowner complaint. The unit has been drilled and completed, however the area surrounding the wellhead is very muddy and not draining. The lease road is in good condition but will be prone to washout during rain events due to the steep grade. The cut bank of the lease road will likely need stabilization. The toe of the slope of the lease road is very wet. I called Anthony Fazio of Pangea and discussed remedies. I also talked to the landowner Scott Boehnlein about his concerns. We agreed that regarding the well area to improve drainage, seeding would be needed. I suggested water bars for the lease road to prevent channeling. I conveyed these issues to Mr. Fazio. He indicated that he would conduct the work within the next few days. I spoke with him again on 10/18/2012 and he indicated the requested work had been completed. I contacted Scott Boehnlein and let him know.
1515421980 2011-01-04 Drill / Deepen / Reopen No Violations Drill depth 3449 Operator encountered Newburg Gas at 3372. Slight odor of sour gas was noticeable by the discharge line. Walked area with gas detector no LEL readings or H2S readings around the pits, rig or area. Took reading 5 feet from the discharge line, no readings. Pit liners were in good repair, physically checked the north east corner of the pits to make sure they had not slipped. Lease road was stable. Site was frozen. Operator has installed drainage controls north end of location.
1552208407 2012-01-04 Drill / Deepen / Reopen No Violations Drill depth 3448. Operator encountered Newburg gas at 3372, drilled stated it was sour gas. Odor noted during inspecting pits. Checked around the pits and the drill pad with gas meter, no LEL or H2S detected. Took readings 5 feet from the discharge of the blow line, no LEL or H2S readings. Operator is drilling on fluid. Pit liners are in good repair, north east corner of the pit liner appeared to have slipped, physically confirmed the condition of the liner, it was found that, it was slack liner folded onto its self. Lease road is stable. Drainage contained on location at the time of inspection.
1628834611 2012-04-26 Preliminary Restoration No Violations My inspection today found the operator actively reclaiming the location. A dozer was working the lease road. I did not enter the site.
