0286422790 2013-06-06 UrbanProduction Wells No Violations My inspection found the well to be equipped with a plunger lift unit. The annulus valve was open The well and tank battery are in good repair and surrounded by a chain link fence. The tank battery consists of two 100 BBL production tanks, a 50 BBL poly brine tank, and a mechanical separator. The production tanks vent stack is plumbed into a 55 BBL charcoal filter. Then it is plumbed up and capped with a spark arrestor. I left a message for the well tender to have a identification sign placed on the tank battery.. The lease road is stable.
0339529840 2011-02-01 Urban Drill / Deepen / Reopen No Violations On site for conductor cement job. Operator drilled to 180 feet with an 14.75 inch bit. Drift in the area 144 feet in depth. Operator set 168 feet of 11.75 inch R-3 32lb pipe. Pipe was set opened. Operator broke circulation, mixed cement and pumped cement until it returned to surface. Operator displaced with fresh water, 17 Bbl return of good cement to surface. Cement water pH was 7. Operator displaced cement to 150 feet. Total of 225 sacks of class A cement used. Operator drilled on fresh water mud.

Perforations were taking fluid, it took a longer time to pressure up then expected. Total scheduled fluid for the job was 55,000 gallons. Operator broke down perforations, balled off and continued fracturing treatment, shut down for 30 minutes then flushed. Job went well, no back side circulation noted. Additives ran CIA-Chek LP 23 gallons, Supersurf 19 gallons, Uniel SF 600 lbs, Leb-10X 1qt, Iron Static 15 gallon, Unislik 30 18 gallons, Bioclear 1000 6 gallons. 33 7/8 perfballs, 1 perfball gun.

Pit of the pit was 6 inches into the gray clay. Pit was lined. Silt fence is up around the site. Culvert in the lease road has been constructed. Topsoil is separated. Permit sign is posted. Permit is posted in the dog house. Operator is planning on spudding well 02/01/2011. Checked Pander Trucking U.I.C. #45 the driver was loading water into the slat water tank. The salt water was from the Bogomany well in Cuyahoga.
1296756363 2011-02-02 Urban Drill / Deepen / Reopen No Violations On site for surface cement. Pits are in good repair. Lease road is stable. Safety fence is up. Drainage controls are in place. Permit is posted in the dog house. Operator had drilled surface hole on fluid. Fresh water tank and brine tank are on location. Operator drilled an 11 inch hole to 325 feet. Surface casing is 8.625 inch 20 pound R-3 new pipe. Casing was set open ended, set 304 feet / 314 feet with the landing joint. Operator broke circulation, mixed cement with one bag of hauls, mixed remaining cement and ran cement. Operator switched over, displaced cement with fresh water, 9Bbls of good cement returned to surface. Operator shut down, closed valves and will wait 8 hours before breaking off the cement head. Roy Poulson and Warren Schonfelt on location from the drilling company and JR Smail. BOP test witness waived, informed driller to perform test and document. Cementing water pH taken the day before pH was 7. No Berea noted.
1297710262 2011-02-07 Urban Drill / Deepen / Reopen No Violations On site for surface long string surface cement. Hole #3 was 7 7/8 TD at 3815. Production pipe was 4.5 inch R-3 new 11.6 lb pipe. Pipe was set with 7 centralizers. Operator broke circulation and ran cement. No lose of circulation during cementing. Cement weight taken during job, cement weight was within limits. Pits were in good repair. Lease road was stable. Site was snow covered.
1298384854 2011-02-15 Urban Drill / Deepen / Reopen No Violations Operator is soldifiying pits at the time of inspection. Operator mixed two loads of fines into the pits. With the cold weather conditions and ice operator order and mixed in a third load of fines. Pits were stiffin up by 1:30pm. Operator will close pits 02/16/2011. Operator was careful not tear liner at the bottom. Drainage contols on location were in good repair. Lease road was frozen and stable. Annulus was open on the gas well.

Five day test reporting was as follows: 19 May 2012 10psi, 20 May 2012 10psi, 21 May 2012 11psi, 22 May 2012 11psi and 23 May 2012 11psi. Well Annulus Pressure test passed. Allowable pressure for the annulus is 92.4psi.

The well was fenced but needed a lock on the man door. Production records for this well were up to date. The tank battery consisted of two 100 BBL production tanks, one 50 BBL poly brine tank and a mechanical separator. Valves on the separator were open and the chart showed production. The tank valves were equipped with flat plugs and the vent stack on the production tank was plumbed into a 55 gallon charcoal filter and topped with a a spark arrestor. The tank battery was diked and fenced with a locked man door. Signage was absent from the well site and needs to be installed at the well and tank battery locations. The lease access was stable. I contacted Mark Turpak from Cutter Exploration about the installation of signage at the well and tank battery and adding a lock on the well fence.
1973658711 2011-07-06 Preliminary Restoration No Violations Well is fenced. Tank battery spacing is correct. Fencing is the correct height. Two 100Bbl tanks are in the batter. There is one man door to the battery, if both tanks are production tanks, another man door will need to be installed. Operator is currently redistributing topsoil. Lease road is stable.
